低强度635nm半导体激光鼻黏膜照射对人体血液流变学影响的临床研究 被引量:5

摘要 目的观察低强度激光鼻黏膜照射对人体血液流变学等的影响,探讨该疗法的安全性和临床应用前景。方法从健康查体人群中,按入选标准筛选出血流变检验异常者84例为实验对象,随机分为实验组和对照组。实验组采用波长635nm、功率15mW、光斑直径4mm,红色铝镓铟磷半导体激光进行鼻腔黏膜照射,每日1次,每次30min,连续15d。对照组仅接受相似红光照射,其余步骤同实验组。血尿标本采集和鼻腔检查分别在照射前、照射结束、撤出照射10d后进行。结果实验组全血低切和高切比黏度在治疗前(基线)、结束时分别为11.44±1.59和10.27±1.11;4.48±0.52和4.18±0.43,差异非常显著(P<0.01;P<0.01)。实验组低切、高切比黏度的95%可信度(95%CI)分别为1.17(0.48~1.54)和0.30(0.18~0.42)。撤出照射10d后,两组差异没有显著性(P>0.05),其疗效的持续性不明显。与此对应,实验组红细胞聚集指数亦有相似变化(P<0.05),并且这几个指标之间有较高的相关性(P<0.05)。激光鼻黏膜照射对血浆比黏度、总胆固醇、甘油三酯、血糖等次要观察指标的影响较弱(P>0.05)。实验中两组受试者生命体征平稳,血尿常规、实验室化验、凝血指标均在正常或安全的范围内;鼻腔检查未发现因激光鼻黏膜照射导致的损伤事件。结论低强度激光鼻黏膜照射能显著降低全血低切比黏度、全血高切比黏度和红细胞聚集,对高粘血症有明显的改善作用。该方法使用安全,对机体干扰小,无明显副作用,故具有较好的临床应用前景。 Objective To observe the change of hemorheological factors and viscosity in blood via nasal low level laser radiation ( NLLLR), and to assess ith efficacy and safety. Methods Eighty-four subjects with hemorheological disorder were divided into trial or control groups randomly by 1 : 1 enveloped sequence. The trial subjects were irradiated by 635 nm red diode laser device for 30 minutes per day for 15 days, meanwhile the control ones received pseudo-laser irradiation in the same time. The blood and urine samples were collected at pre- and post-irradiation and 10 days after the irradiation. Results The main parameter values about erythrocyte aggregation and erythrocyte deformability in trial group were 10. 27 ± 1.11 and 4. 18± 0.43 after NLLLR, respectively, which were remarkably lower than that of the baseline ( 11.44 ± 1.59 and 4. 48 ± 0. 52 , P 〈 0. 01 and P 〈 0. 01 ). The values likewise had 95% CI 1.17 ( 0. 80-1.54) and 0. 30 ( 0. 18-0. 42 ) , respectively. The efficacy of NLLLR, however, did not maintain after 10 days, for no significant difference of erythrocyte aggregation and erythrocyte deformability between 2 groups was found ( P 〉 0. 05 ). The results had indicated that NLLLR had a weak effect on plasma viscosity, total cholesterol, triglycerin, and glucose as well ( P 〉 0. 05 ). Blood pressure, heart rate, routine tests, hepatic and renal functions and nasoscope were within normal range during the trial course in 2 groups. Conclusions NLLLR could be a useful and safe teehnique to improve erythrocyte aggregation and erythrocyte deformability, and prevent the occurrence of hemorheological disorder.
