膀胱移行细胞癌中树突状细胞的免疫状态变化及意义

摘要 目的:通过分析膀胱移行细胞癌中浸润性树突状细胞(DC)的免疫表型变化,探讨其在肿瘤微环境中的功能状态。方法:选择未经任何治疗的初发膀胱移行细胞癌患者标本35例作为实验组,正常膀胱组织作为对照组。通过免疫组化技术标记组织中树突状细胞的CD1a、HLA-DR和CD86抗原,观察两种组织中的DC表达3种抗原的状况,结果进行统计学分析。结果:所有病例均未见明显CD86+DC,但均有CD1a+DC,其在膀胱癌组织中、癌旁组织中的浸润程度均低于正常膀胱组织,差异均有统计学意义(P<0.05),在膀胱癌组中有22例癌实质内的DC表达HLA-DR抗原,阳性表达率为48.6%,与正常膀胱组织比,差异无统计学意义(P>0.05)。结论:膀胱移行细胞癌组织中的DC,其浸润程度下降并存在功能成熟障碍。 Objective: To elucidate the functional status of dendritic cells (DCs) in the tissue of transitional bladder cancer by analyzing of them. Methods:35 specimens from transitional bladder cancer cases primarily treated with surgery were selected as test group. In addition,35 specimens of normal bladder tissue were used as control. Distribution of DC expressing CD1a^+ , HLA - DR + and CD86 ^+ in tumor tissue and normal tissue was observed by immunohistochemistry. The density of DC and the rate of DC expressing HLA- DR in Renal cell carcinoma group and control were statistically compared. Results:There was no CD86^+ DC in all cancers, but CD1a ^+ DC was found in all cancers. The density of CD1a ^+ DC in tumor tissue was significantly lower than that in normal tissue( P 〈 0.05). HLA- DR antigen expressed on the surface of DC in tumoral tissue of 17 cases carcinoma specimens and in normal tissue of 22 cases. The rate of HLA - DR positive expression of TLDC had no statistic significance between the two groups. Conclusion: The lower density of DC infiltrating in tumor tissue might reflect the microenvimmental immunodeficiency of hosts with transitional bladder cancer , and the functional mature of DC might be inhibited by the immunosuppressive action of transitional bladder cancer.
