摘要
目的应用超声二维斑点追踪显像技术探讨不同年龄正常成人左室扭转与径向位移的关系。方法健康成年志愿者121例,按年龄分为青年组(18~45岁)、中年组(46~64岁)和老年组(≥65岁)。取胸骨旁左室心尖和心底短轴观对左室各水平旋转及径向位移进行测量分析,计算出左室扭转与左室径向位移,构建左室扭转-位移环,并比较不同年龄组的差别。结果三组左室心尖部与心底部心肌旋转角度及各节段径向位移曲线随心动周期规律变化。三组左室扭转-位移环均呈“8”字构型。收缩期及等容舒张期左室扭转与径向位移均呈线性相关;随年龄增加,收缩期扭转-位移环的变化斜率明显增加,差异有统计学意义(P〈0.05);与青年组比较,中年组及老年组等容舒张期扭转-位移环的变化斜率均明显增加,差异均有统计学意义(P〈O.05),但中年组与老年组间比较差异无统计学意义(P〉0.05)。结论应用超声二维斑点追踪显像技术可无创性构建左室扭转-位移环,年龄影响收缩期左室扭转-位移环。
Objective To establish left ventricular(LV) twist-displacement loops on health population and relationship between twist and displacement by two-dimensional ultrasound speckle-tracking imaging (STI). Methods One hundred and twenty-one healthy volunteers were divided into three groups by age: younger group ( 19 ~45 years old), middle age group (46~ 64 years old) and old group (≥65 years old). Basal and apical short-axis images of left ventricular were acquired to analysis LV rotation (LVrot) and radial displacement. LV twist and LV displacement were calculated, then LV twist-displacement loop was established and the differences among groups were compared. Results Among 3 groups, both rotation angles of apical and basal myocardium, and radial displacements on regional myocardium had regular changes. Twist-displacement loops were characterized by a figure of 8 configuration. It was a linear relation between twist and displacement during systole and isovolumic relaxation time(IVRT). With aging, the slope of systolic loop increased significantly( P 〈0. 05). Compared with the young groups, the slope of IVRT loop was significantly increased in the middle and old aged groups( P 〈0.05); whereas no difference between the middle and old aged groups( P 〈0.05). Conclusions LV twist-displacement loops can be constructed noninvasively by 2-dimensional ultrasound STI imaging. The assessment of twist-displacement loop may contribute to the evaluation of the LV function in daily clinical practice.
