斑点追踪显像技术评价原发性高血压病患者早期左心室旋转及扭转功能 被引量:6

摘要 目的探讨斑点追踪显像(speckle tracking imaging,STI)评价原发性高血压(EH)患者早期左心室旋转和扭转运动改变的价值。方法对35例左心房、左心室正常构型的EH患者(高血压组)和35例健康者(对照组),经胸采集左心室短轴心尖水平和心底水平的二维图像,计算心底水平旋转角度峰值(MV-Prot)、心尖水平旋转角度峰值(AP-Prot)、扭转角度峰值(Ptw)。结果两组受检者心尖部旋转角度均显著大于心底部旋转角度,差异均有统计学意义(P<0.01);但两组达峰时间比较,差异无统计学意义(P>0.05);高血压组患者心尖部旋转角度显著大于对照组,差异有统计学意义(P<0.05)。结论 EH患者早期左室整体心肌扭转运动主要表现为逆时针方向扭转,扭转运动增强。 Objective To observe the value of speckle tracking imaging (STI) in assessing early changes of left ventricular rotation and twist in patients with essential hypertension (EH) . Methods 35 EH patients with normal left atrial and left ventricular and 35 healthy controls were involved into the study. 2 - D images were acquired from the mitral valve and apex of the left ventricular. Peak rotation on basal plane (MV -Prot), apical plane (AP- Prot) and peak twist (Ptw) were measured. Results In the two groups, AP ' Prot were significantly greater than MV - Prot (P 〈 0. 01 ), but the peak time between the two groups was not significantly different (P 〉 0. 05 ) . AP - Prot in EH group was significantly greater than that in control group (P 〈 0. 05) . Conclusion The motion character of early left ventricular myocardium of EH is mainly counter - clockwise twisting, and Ptw is increased.
