失眠合并抑郁患者的血清甲状腺素水平检测 被引量:7

The role of serum thyroxine in patients with insomnia and depression

摘要 目的调查失眠合并抑郁患者中的血清甲状腺素水平。方法选取2015年6月~2017年6月期间我院收治的138例失眠患者为研究对象,依据患者汉密尔顿抑郁量表(HAMD)评分(评分≥8分为抑郁症)分为抑郁组(n=35)和非抑郁组(n=103)。两组患者均给予帕罗西汀联合阿普唑仑治疗,共治疗6周。比较抑郁组与非抑郁组患者的血清甲状腺素水平,分析失眠抑郁患者HAMD评分与血清甲状腺素水平的相关性,并比较治疗前后抑郁组血清甲状腺素水平。结果两组患者的血清三碘甲状腺原氨酸(T3)、四碘甲状腺原氨酸(T4)和游离四碘甲状腺原氨酸(FT4)水平比较差异无统计学意义(P>0.05);抑郁组患者的血清游离三碘甲状腺原氨酸(FT3)水平低于非抑郁组,血清促甲状腺激素(TSH)水平高于非抑郁组(P<0.05)。抑郁组患者HAMD评分为(15.28±2.77)分,非抑郁组患者HAMD评分为(4.92±1.22)分。抑郁组患者的HAMD评分与血清TSH和T3水平表现出负相关(P<0.05)。治疗前后抑郁组患者的血清T3、T4和FT4水平比较明显差异(P>0.05);治疗后较治疗前抑郁组患者的血清FT3水平升高,TSH水平降低(P<0.05)。结论失眠患者的抑郁情绪会导致其血清甲状腺素水平紊乱,改善失眠症状能够改善甲状腺素水平,改善甲状腺功能。 Objective To explore the role of serum thyroxine in patients with insomnia and depression.Method A total of 138 patients with insomnia admitted to our hospital from June 2015 to June 2017 were selected as research objects,and were divided into the depression group(n=35)and the non-depression group(n=103)according to the Hamilton depression scale(HAMD)score(score≥8 divided into depression).Patients in both groups were treated with paroxetine combined with alprazolam for a total of 6 weeks.Serum thyroxine levels were compared between depressed and non-depressed patients.The correlation between HAMD scores and serum thyroxine levels in patients with insomnia was analyzed.Serum thyroxine levels in the depression group before and after treatment were compared,analysis of thyroid function.Results There were no significant differences in serum levels of triiodothyronine(T3),tetraiodothyronine(T4)and free tetraiodothyronine(FT4)between the two groups(P>0.05).Depression The serum free triiodothyronine(FT3)level was lower in the group than in the non-depressed group,and the serum thyroid stimulating hormone(TSH)level was higher than that in the non-depressed group(P<0.05).The HAMD score was(15.28±2.77)in the depression group and(4.92±1.22)in the non-depressed group.The HAMD scores of patients in the depression group were negatively correlated with serum TSH and T3 levels(P<0.05).There was no significant difference in serum T3,T4 and FT4 levels between the patients before and after treatment(P>0.05).After treatment,the serum FT3 level and TSH level were lower than those in the pre-treatment depression group,and the difference was statistically significant(P<0.05).Conclusion Depressive mood in patients with insomnia can lead to disorder of serum thyroxine levels,and improving symptoms of insomnia can improve thyroxine levels,improve thyroid function.
