摘要
目的分析抑郁症治疗措施对患者甲状腺激素水平的影响。方法选取2019年1月—2020年12月该院收治的抑郁症患者400例为研究对象,所有患者均接受抗抑郁症药物治疗。分别于治疗前、治疗4、8、12周后应用汉密尔顿抑郁评分量表(HDMD-17)对患者进行评分;并根据患者治疗情况将患者分为治疗有效组以及无效组,对患者血清三碘甲腺原氨酸(T3)、血清甲状腺素(T4)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、血清促甲状腺素(TSH)情况进行测定。结果治疗2、4、8周时HDMD-17评分结果分别为(25.18±2.93)分、(21.15±2.23)分、(14.23±3.06)分,均显著低于治疗前,差异有统计学意义(P<0.05)。治疗8周后,共有291例患者判定治疗有效,纳入治疗有效组,109例患者判定治疗无效,纳入治疗无效组。治疗有效组患者治疗后T3、T4、FT3、FT4水平分别为(1.22±0.52)nmol/L、(56.82±5.48)nmol/L、(3.25±0.43)pmol/L、(8.61±0.68)pmol/L,均显著高于治疗前,差异有统计学意义(P<0.05);TSH水平(5.02±0.47)mIU/mL显著低于治疗前,差异有统计学意义(P<0.05);治疗无效组治疗前后T3、T4、FT3、FT4以及TSH水平比较差异无统计学意义(P>0.05);治疗后,有效组与无效组甲状腺相关水平比较差异有统计学意义(P<0.05)。结论采用5-羟色胺摄取抑制剂对抑郁症患者进行治疗,可对患者甲状腺相关激素水平产生一定影响,对患者甲状腺相关激素水平进行检测可利于对治疗效果进行判定以及指导临床调整治疗方案。
Objective To analyze the effect of depression treatment on thyroid hormone level.Methods 400 patients with depression treated in the hospital from January 2019 to December 2020 were selected as the research object,and all patients received antidepressant medications.The patients were scored using the Hamilton Depression Rating Scale(HDMD-17)before treatment,4,8,and 12 weeks after treatment.According to the patient's treatment,the patients are divided into effective treatment group and ineffective group.The patient’s serum triiodothyronine(T3),serum thyroxine(T4),free triiodothyronine(FT3),free thyroxine(FT4),and serum thyroxine(TSH)were measured.Results The HDMD-17 scores at 2,4,and 8 weeks of treatment were(25.18±2.93)points,(21.15±2.23)points,and(14.23±3.06)points,respectively,which were significantly lower than before treatment,and the difference was statistically significant(P<0.05).After 8 weeks of treatment,a total of 291 patients were judged to be effective and included in the effective treatment group,and 109 patients were judged to be ineffective and included in the treatment ineffective group.The levels of T3,T4,FT3 and FT4 in the treatment effective group were(1.22±0.52)nmol/L,(56.82±5.48)nmol/L,(3.25±0.43)pmol/L,and(8.61±0.68)pmol/L,respectively,both were significantly higher than before treatment,and the difference was statistically significant(P<0.05),and the TSH level(5.02±0.47)mIU/mL was significantly lower than before treatment,and the difference was statistically significant(P<0.05).There was no significant difference in T3,T4,FT3,FT4 and TSH levels before and after treatment in the treatment-ineffective group(P>0.05).After treatment,the thyroid-related levels of the effective group and the ineffective group were significantly different,and the difference was statistically significant(P<0.05).Conclusion The treatment of depression patients with 5-hydroxytryptamine uptake inhibitor can have a certain effect on thyroid related hormone level,the detection of thyroid related hormone level
