摘要
中医住院医师规范化培训(简称住培)是加强中医临床人才队伍建设的有效途径。自2014年启动实施以来,已在全国基本建立了中医住培制度,成功培养了一批优秀的中医临床人才队伍。但不可忽视的是中医住培仍处于起步阶段,存在诸多问题,其中以中医临床思维弱化、“西化”、僵化等现象尤为突出。以象、阴阳、五行、精气学说为基础的中医核心观念,是中医思维的关键内容,也是中医临床思维的核心,是中医特色所在。但目前培训实践中中医临床思维的培养内容、培养方式尚有诸多欠缺。该文从中医临床思维的内涵及中医临床思维培养模式设计的角度出发,提出中医住培应当通过读经典、多临床、加强考核、师资培训、医教协同等措施,强化对中医核心观念的把握和培养,突出中医特色,建设高素质的中医临床人才队伍,促进中医药事业的传承与发展。
The construction of Traditional Chinese Medicine(TCM)clinical talents could be effectively strengthened by the standardized training of TCM residents(hereinafter referred to as TCM residency training).Since the project was launched in 2014,the TCM residency training system has been basically established nationwide,and a numerous excellent TCM clinical talent teams have been successfully trained.However,it cannot be ignored that TCM residency training is still in its initial stage and there are still many problems,among which the phenomena of weakening,"westernization"and rigidity of clinical thinking in TCM are particularly prominent.The essential concept of TCM based on the doctrines of manifestation,Yin and Yang,five elements and vital essence is the key content of TCM thinking,and also the core of TCM clinical thinking,which is the characteristic of TCM.However,there are still many shortcomings involved in the training contents and training methods of TCM clinical thinking in the training practice of residency training bases.Therefore,from the perspective of the connotation of TCM clinical thinking and the design of TCM clinical thinking training mode,it is proposed in the article that TCM residency training should strengthen the grasp and cultivation of core concepts of TCM,highlight the characteristics of TCM,build a high-quality TCM clinical talent team,and promote the inheritance and development of TCM by reading classics,participating in more clinical practice,strengthening assessment,teacher training,medical-teaching collaboration and other measures.
