摘要
目的探讨中性粒细胞明胶酶相关脂质运载蛋白(NGAL)与类风湿因子(RF)联合检测对类风湿关节炎(RA)的诊断价值。方法选择2017年3月至2018年10月绵阳市中心医院65例RA患者(RA组)、65例非RA患者(非RA组)和66名健康体检者(健康对照组)进行NGAL和RF检测,采用免疫透射比浊终点法检测NGAL,透射比浊法检测RF。结果RA组RF和NGAL水平为117.00(23.60~394.00)IU/mL、167.20(132.00~202.00)μg/L,均高于非RA组11.20(11.00~12.50)IU/mL、132.80(102.00~178.10)μg/L和健康对照组11.00(10.40~71.50)IU/mL、117.45(95.55~139.75)μg/L,差异有统计学意义(P<0.05)。RF or NGAL联合检测的灵敏度95.38%,明显高于RF(80.00%)和NGAL(64.60%)单独检测,差异有统计学意义(P<0.05);RF+NGAL联合检测的特异度为100.00%,高于NGAL单独检测(86.40%)(P<0.05),与RA组单独检测(92.40%)差异无统计学意义。NGAL、RF及联合检测诊断RA的ROC曲线下面积分别为0.796、0.904和0.931,NGAL与RF呈正相关(r=0.353,P=0.000)。结论NGAL表达水平可用于诊断RA,且与RF联合检测可提高RA的检出效率,具有较高的诊断价值。
Objective To investigate the value of combined detection of neutrophil gelatinase associated lipocalin(NGAL)and rheumatoid factor(RF)in rheumatoid arthritis(RA).Method NGAL and RF in 65 cases of RA,65 cases of non-RA and 66 cases of healthy subjects were detected by the immunotransmitting turbidity endpoint and Nephelometry.Results The RF and NGAL levels in the RA group 117.00(23.60~394.00)IU/mL,167.20(132.00~202.00)μg/L were higher than those in the non-RA group 11.20(11.00~12.50)IU/mL,132.80(102.00~178.10)μg/L and the healthy control group 11.00(10.40~71.50)IU/mL,117.45(95.55~139.75)μg/L(P<0.05).The sensitivity of RF or NGAL combined detection was95.38%,which was significantly higher than that of RF(80.00%)and NGAL(64.60%)alone,the difference was statistically significant(P<0.05);the specificity of RF+NGAL combined detection was 100.00%,which was higher than the NGAL alone(86.40%)(P<0.05),and there was no significant difference with the RA group alone(92.40%)(P>0.05).The area under the ROC curve of NGAL,RF and Joint detection to diagnostic RA were 0.796,0.904 and 0.931,respectively.NGAL was positively correlated with RF(r=0.353,P=0.000).Conclusion NGAL could be used for the diagnosis of RA,and the combined detection with RF could improve the detection efficiency of RA,and had a high diagnostic value.
