摘要
目的：探讨氟康唑口服与硝酸咪康唑阴道给药单用或联用治疗重度 VVC 临床疗效及安全性差异。方法：研究对象选取我院2012年4月～2015年4月收治重度 VVC 患者共90例，采用随机区组法分为 A 组（30例）、B 组（30例）及 C 组（30例），分别给予氟康唑口服单用，硝酸咪康唑阴道给药单用及两者联用治疗；比较三组患者临床疗效，真菌清除率、治疗前后阴道分泌物炎性因子水平，随访复发率及不良反应发生率等。结果：C 组患者临床疗效显著优于 A、B 组，差异具有显著性；C 组患者真菌清除率显著高于 A、B 组，差异具有显著性；C 组患者治疗后阴道分泌物炎性因子水平均显著低于 A、B 组，差异具有显著性；C 组患者随访复发率显著低于 A、B 组，差异具有显著性；同时三组患者不良反应发生率比较差异无显著性。结论：氟康唑口服与硝酸咪康唑阴道给药联用治疗重度 VVC 可有效减轻临床症状体征，提高真菌清除效果，抑制阴道炎性反应水平，降低随访复发风险，且未增加不良反应发生几率，效果优于两药单用。
Objective To investigate the clinical effects and safety differences of fluconazole orally therapy and miconazole nitrate vaginal therapy used alone or combination in the treatment of severe VVC. Methods 90 patients with severe VVC were chosen in the period from April 2012 to April 2015 in our hospital and randomly divided into both group including A group (30 patients) with fluconazole orally therapy used alone, B group (30 patients) with miconazole nitrate vaginal therapy used alone and C group (30 patients) with fluconazole orally therapy combined with miconazole nitrate vaginal therapy; and the clinical ef-ficacy, fungal clearance rate, the level of inflammatory factors of vaginal secretions, recurrence rate with follow-up and the ad-verse reactions incidence of both groups were compared. Results The clinical efficacy of C group was significantly better than A group and B group. The fungal clearance rate of C group was significantly better than A group and B group. The level of inflam-matory factors of vaginal secretions after treatment of C group was significantly lower than A group and B group . The recurrence rate with follow-up of C group was significantly lower than A group and B group. The adverse reactions incidence of C group was significantly lower than A group and B group. Conclusion Compared with fluconazole orally therapy and miconazole nitrate vaginal therapy used alone, Combination therapy in the treatment of severe VVC can efficiently relieve the clinical symptoms and signs, higher the fungal clearance effect, inhibit the inflammatory level of vaginal reaction, reduce the recurrence risk and not increase the adverse reactions risk.
