异丙酚用于小儿心脏术后机械通气时镇静作用80例临床观察 被引量:6

摘要 目的比较异丙酚与吗啡、羟丁酸钠用于小儿心脏手术后机械通气时的镇静作用及对血流动力学的影响.方法 80例先天性心脏病手术患儿随机分为异丙酚(PP)组和吗啡、羟丁酸钠(MR)组,每组40例.PP组全程静脉输注异丙酚l0~50μg·kg-1·min-镇静,MR组用吗啡0.1~0.2mg/kg、羟丁酸钠120mg/kg镇静,观察镇静期间的镇静深度、相关时间及血流动力学变化.结果 PP组镇静评分、停药至脱机及拔管时间、拔管后嗜睡率明显低于MR组(P<0.05),但满意镇静时间长于MR组(P<0.05);pP组循环较MR组稳定,心率、血压均低于MR组(P<0.05).结论异丙酚静脉输注法用于小儿心脏手术后机械通气时的镇静,效果优于吗啡、羟丁酸钠. ObjectiveTo compare sedative action of propofol, morphine, sodium r-hydroxy-butyrate for mechanical ventilation in heart postoperative children and influence on blood dynamics.Methods80 cases of postoperative patients with congenital heart disease were randomly divided into propofol(PP) group, morpline and sodium r-hydroxy-butyrate(MR) group, each having 20 cases. The cases in PP group were injected intravenously with 10~50μg·kg^(-1)·min^(-1) propofol. The cases in MR group were injected with 0.1~0.2mg/kg morphine and 120mg/kg sodium r-hydroxy-butyrate. Sedation depth, relation time and dynamic change of blood during sedation were observed.ResultsSedation score, time from stoping administration of drug to separating from respiratory aparatus and pulling out catheter, lethargy rate after pulling out catheter in PP group were obvioubly lower than that of MP group( P <(0.05)), but satisfied sedation time was longer than that of MP group( P <(0.05)). The circulation in PP group was more constant than that of MP group. The heart rate and blood pressure were lower than that of MR group( P <(0.05)).ConclusionThe effect of propofol transfusion intravenously is better than that of morphine and sodium r-hydroxy-butyrate for sedation in heart postoperative mechanical ventilation and worth application and dissemination in ICU.
