摘要
探讨地方性和AIDS相关Kaposi肉瘤的组织发生。方法:以FⅧRAg、CD34、Vimentin、Actin和Desmin作一抗,对21例地方性和9例AIDS相关Kaposi肉瘤作了免疫组化标记。根据组织学特征,把Kaposi肉瘤分为血管瘤样型、混合型和梭形细胞型。结果:FⅧRAg和CD34在病变周围的正常血管强阳性表达,淋巴管不表达。FⅧRAg在Kaposi肉瘤组织内血管成分表达强,裂隙样结构和梭形细胞表达弱或不表达;CD34在血管成分和梭形细胞皆呈阳性或强阳性表达;Vimentin在血管成分和梭形细胞皆呈阳性或强阳性表达;Actin在病变组织内的厚壁血管表达,薄壁血管、裂隙样结构和梭形细胞不表达。Desmin在Kaposi肉瘤组织表达阴性。组织形态学显示,病变组织内的血管样和腔隙样结构内皆充满血细胞,为血管内皮细胞起源的证据。结论:Kaposi肉瘤起源于血管内皮细胞,梭形细胞和血管内皮细胞同源。
Objective:To explore the histogenesis in endemic and AIDS-ralated Kaposi's sarcoma (KS).Methods:Imunohistochemical methods in21cases of endemic and9cases of AIDS-related KS were performed using FⅧRAg,CD34,Vimentin,Actin,and Desmin antibodies with S-P methods.KS was classified into three types,i.e.,angiomatous type,mixed type,and spindle cell type depending on the histological characteristics.Results:FⅧRAg and CD34label-ings were positive in normal blood vessel,but negative in lymphatics in the surrounding tissues of KS lesions.FⅧRAg labeling showed that the well-formed vascular areas of KS were stained strongly,but cleft-like structures and spindle cell areas were poor or negative.CD34and Vi-mentin staining were positive or strong positive both in vascular structures and spindle cells.Actin labeling was positive in well-differentiated vasculars and no staining in spindle cell areas.The features of KS in histology suggeste that KS cells derive from the endothelial cells of blood vessel based on the lumen structures filled with blood cells in the KS lesions.Conclusions:KS cells originate from the endothelial cells of blood vessel.The vascular components and the spindle cells originate from the same cell clone.
