摘要
目的:探讨生肌象皮膏对糖尿病足溃疡大鼠创面愈合及Notch信号通路的影响。方法:将80只SD大鼠随机分为正常对照组、模型组、凡士林组(10 mg·kg-1)、生肌象皮膏组(20 mg·kg-1),每组20只。模型组、凡士林组和生肌象皮膏组构建糖尿病足溃疡大鼠创面模型,正常对照组构建单纯性足溃疡大鼠创面模型。建模成功后,凡士林组和生肌象皮膏组溃疡创面伤口外敷相应药物,正常对照组和模型组大鼠溃疡创面伤口不予处理。采用免疫组织化学法检测溃疡创面伤口组织成纤维细胞数、毛细血管表达情况;Notch受体1(Notch1)、转化生长因子-β1(TGF-β1) mRNA及蛋白表达情况;白细胞介素-2(IL-2)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)表达情况。结果:与正常对照组比较,模型组新生毛细血管数密度、成纤维细胞数水平显著降低,创面组织Notch1、TGF-β1mRNA及蛋白表达、IL-2、IL-6、TNF-α水平显著升高(P<0.05);与模型组比较,凡士林组和生肌象皮膏组新生毛细血管数密度、成纤维细胞数水平显著升高,创面组织Notch1、TGF-β1mRNA及蛋白表达、IL-2、IL-6、TNF-α水平显著降低(P<0.05);凡士林组和生肌象皮膏组比较差异无统计学意义(P>0.05)。正常对照组创面组织见炎症细胞浸润,间质充满大量成纤维细胞,创面组织结构正常,无炎症细胞浸润,间质无异常;模型组见大量中性细胞及嗜酸性细胞浸润,成纤维细胞及毛细血管较少;凡士林组和生肌象皮膏组仍可见中性细胞及嗜酸性细胞浸润,但数量明显较模型组减少,成纤维细胞及毛细血管明显增多。结论:生肌象皮膏能减轻大鼠糖尿病足溃疡创面炎症反应,促进创面预后;生肌象皮膏能抑制Notch1、TGF-β1 mRNA及蛋白的表达进而抑制糖尿病大鼠足溃疡创面炎症反应,其机制与抑制IL-2、IL-6、TNF-α有关。
Objective:To investigate the effect of Shengji Xiangpi ointment on wound healing and Notch signaling pathway in diabetic foot ulcer rats.Methods:Totally 80 SD rats were randomly divided into the normal control group,the model group,vaseline group(10 mg·kg-1)and Shengji Xiangpi ointment group(20 mg·kg-1)with 20 ones in each group.The wound model of diabetic foot ulcer was established in the model group,vaseline group and Shengji Xiangpi ointment group,and the wound model of simple foot ulcer was established in the normal control group.After the successful modeling,vaseline group and Shengji Xiangpi ointment group received the corresponding treatment,and no treatment was given to the ulcer wound in the normal control group and the model group.At the end of experiment,the number of fibroblasts,and the expressions of capillary,mRNA and protein of Notch1 and TGF-β1,as well as the expressions of IL-2,IL-6 and TNF-αwere detected in the wound tissue of ulcer wounds.Results:Compared with those in the normal control group,the number of fibroblasts and capillary expression in the model group decreased significantly,while the mRNA and protein expression levels of Notch1 and TGF-β1 and the expressions of IL-2,IL-6 and TNF-αincreased significantly(P<0.05).Compared with those in the model group,the number of fibroblasts and capillary expression in vaseline group and Shengji Xiangpi ointment group increased significantly,while the mRNA and protein expression levels of Notch1 and TGF-β1,and the expressions of IL-2,IL-6 and TNF-αdecreased significantly(P<0.05).There were no significant differences between vaseline group and Shengji Xiangpi ointment group(P>0.05).In the normal control group,the inflammatory cells infiltrated in the wound tissue,the interstitial was filled with a large number of fibroblasts,the wound tissue structure was normal,no inflammatory cells infiltrated,and the interstitial was normal.The model group showed a large number of neutrophils and eosinophils infiltrating,many fibroblasts and fewer capillar
