摘要
目的探讨生肌散、蜂蜜外敷联合用于单纯性肛瘘患者术后创面治疗的效果。方法选取2018年6月~2019年4月在本院治疗的176例低位单纯性肛瘘患者随机分为两组:对照组(n=88)给予生肌散外涂治疗,观察组(n=88)给予生肌散配伍蜂蜜外敷治疗。观察患者术后创面疼痛、组织形态学改善及创面愈合情况等。结果术后第14天,观察组疼痛、出血、渗液及肛缘水肿、排尿困难、排便困难并发症积分均显著低于对照组(P<0.05);术后第7天、第14天,观察组红活创面比例分别为68.18%、95.45%,对照组分别为36.36%、60.23%,观察组均显著高于对照组(χ^2=17.855、31.655,P<0.05);观察组术后第7天巨噬细胞比例、成纤维细胞数、新生毛细血管数均显著高于对照组(t=16.593、13.410、18.602,P<0.05),术后第14天各指标也均显著高于对照组(t=8.539、25.639、8.175,P<0.05);观察组患者术后SF-36量表中7个项目评分均显著高于对照组(P<0.05)。结论生肌散配伍蜂蜜应用于单纯性肛瘘患者术后创面治疗,可有效抑制创面细菌生长,减轻术后并发症,有助于患者术后生活质量获得更好改善。
Objective To investigate the effect of Shengji San and honey external application in the treatment of postoperative wounds in patients with simple anal fistula.Methods A total of 176 patients with low-grade simple anal fistula who were treated in our hospital from June 2018 to April 2019 were randomly divided into two groups.The control group(n=88)was given the treatment of Shengji San external application,and the observation group(n=88)was treated with Shengji powder and honey.The postoperative wound pain,histomorphological improvement and wound healing of patients after surgery were observed.Results On the 14th day after surgery,the pain,bleeding,exudate and anal edema,dysuria,and difficulty in defecation were significantly lower in the observation group than those in the control group(P<0.05).On the 7th and 14th day after surgery,the proportion of red and live wounds in the observation group was 68.18%and 95.45%,respectively,and those of the control group was 36.36%and 60.23%,respectively.The proportion of red and live wounds in the observation group was significantly higher than that in the control group(χ^2=17.855,31.655,P<0.05).The proportion of phagocytes,the number of fibroblasts,and the number of new capillaries in the observation group were significantly higher than those of the control group(t=16.593,13.410,18.602,P<0.05).On the 14th day after surgery,the indexes in the observation group were also significantly higher than those of the control group(t=8.539,25.639,8.175,P<0.05).The scores of 7 items in the SF-36 scale of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Shengji San combined with honey for the treatment of postoperative wounds in patients with simple anal fistula can effectively inhibit the growth of wound bacteria,reduce postoperative complications,and help patients to improve their quality of life after surgery.
