摘要 目的IgA肾病(IgAnephropathy,IgAN)是世界范围内较常见的原发性肾小球肾炎,本文旨在研究Notch信号转导通路在IgA肾病患者血液以及病理组织中表达的情况,从而探讨Notch信号通路与IgA肾病的关系。方法实验组来自包头医学院第一附属医院肾脏内科2013年1月至2014年10月确诊的IgA肾病患者36例(男∶女=16∶20,年龄20~64岁),根据IgA肾病牛津病理分型肾小管萎缩或间质纤维化(T)结果将实验组分为3组,即N1、N2、N3组。N1组(n=12,男∶女=6∶6),N2组(n=17,男∶女=7∶10),N3组(n=7,男∶女=3∶4)。对照组为包头医学院第一附属医院泌尿外科肾肿瘤(远离肿瘤的瘤旁组织)患者(经光镜证实为正常肾组织)(n=5,男∶女=3∶2)。运用酶联免疫吸附法检测患者血液中的Notch1、Jagged1水平。运用免疫组织化学方法检测肾组织中Notch1、Jagged1、Hey1、Hes1的表达。所有实验数据均采用SPSS17.0软件系统进行统计分析。计量资料以“x±s”表示,检验为正态方差齐,用方差分析(ANOOA),非正态或方差不齐用非参数检验,多组间比较用Kruskal-WallisH检验,P<0.05表示差异有统计学意义。结果血Notch1、Jagged1水平在IgA肾病各组N1、N2、N3与对照组比较均增高,差异有统计学意义(P<0.05)。在N3中表达最高,在N2组次之,各组间差异有统计学意义(P<0.05)。免疫组化结果显示:Notch1、Jagged1、Hey1、Hes1的表达在对照组和实验组N1、N2、N3各组中均有表达;在对照组,Notch1和Jagged1、Hey1、Hes1仅有极少量表达;实验组较对照组表达增强,在IgA肾病各组N1、N2、N3中,随着病情的进展,Notch1和Jagged1、Hey1、Hes1的表达越来越强,至N3组时最强。结论在IgA肾病患者中,Notch1、Jagged1、Hey1和Hes1在肾组织中均有表达,且随着肾间质纤维化程度的加重,表达逐渐增强。Notch1、Jagged1在外周血中的水平也随之增多,提示IgA肾病患者血中的监测结果与病理组织的监测结果相一致,肾� Objective IgA nephropathy is a common primary glomerulonephritis worldwide. The purpose of this study is to investigate the expression of Notch signal transduction pathway in blood and pathological tissues of patients with IgA nephropathy, and to explore the relationship be- tween Notch signal transduction pathway and IgA nephropathy. Methods Experimental groups are from baotou medical school first affiliated hospital, kidney internal medicine, from January 2013 to October 2014, 36 patients with the diagnosis of IgA nephropathy (sex ratio=16∶20, age 20-64). According to the Oxford IgAN pathological classification,the experimental groups can be divided into 3 groups, namely, N1, N2, N3 group. According to the results of pathological renal tubular atrophy and interstitial fibrosis (T) as a result, N1 group (n=12 , sex ratio=6∶6), N2 group (n=17, sex ratio=7∶10), N3 group (n=7, sex ratio=3∶4). The control group was patients with renal tumors in urology department of the first affiliated hospital of baotou medical college (confirmed by using the normal renal tissue) as control group (n=5, sex ratio=3∶2). Using enzyme-linked immunosorbent method to detect patients with IgAN, Jagged 1 Notch 1 levels in the blood. Using mmunohistochemical method to detect renal tissues the expression of Notch 1, Jagged 1, Hey 1 and Hes 1. All the experimental data are used SPSS 17. 0 software system were analyzed. Measurement data to mean and standard deviation (x±s), according to test for normal variance, with analysis of variance (ANOOA), non-normal or not together with nonparametric test, variance between groups compared with Kruskal Wallis H - check,P<0.05 indicates that the difference was statistically significant. Results Enzyme-linked immunosorbent method detection results: blood, Jagged1 Notch 1 level in IgA nephropathy group N1, N2 and N3 compared with control group were higher, the difference was statistically significant (P<0.05).
