摘要
目的探究糖尿病创面基质金属蛋白酶9(MMP-9)对血管内皮生长因子(VEGF)的影响。方法选取2014年7月至2015年7月华中科技大学同济医学院附属梨园医院住院患者为研究对象,其中糖尿病足患者19例,骨科非糖尿病患者7例,将所招募的26例患者分为4组:糖尿病足患者创面伴随大量细菌>1×10~6个/g组织为A组7例,糖尿病足患者创面伴随少量细菌<1×10~6个/g组织为B组12例,糖尿病无创面组织(距糖尿病足溃疡创缘大于2 cm处皮肤)为C组6例,以及骨科非糖尿病患者组织为D组7例。MMP-9和VEGF的表达采用荧光定量聚合酶链反应(RT-qPCR)和蛋白质免疫印迹分析法检测。组间差异用单因素方差分析并经过Scheffe检验,组间两两比较采用q检验。结果 A、B、C、D组RT-qPCR检测的MMP-9相对表达量分别为1.14±0.12、0.60±0.08、0.39±0.09、0.13±0.06,比较差异有统计学意义(F=43.92,P<0.05);RT-qPCR检测的VEGF相对表达量分别为1.01±0.22、3.55±0.97、5.72±0.55、6.92±1.55,比较差异有统计学意义(F=40.74,P<0.05);蛋白质免疫印迹分析法检测结果与RT-PCR结果一致。结论不同糖尿病创面组织中MMP-9的表达增高而VGEF的表达降低。
Objective The aim of this study was to explore the effect of matrix metalloproteinases 9(MMP-9) on the expression of vascular endothelial growth factor(VEGF) in diabetic wound. Methods Between July 2014 and July 2015,patients were enrolled in Liyuan Hospital,Affiliated of Tongji Medical College,Huazhong University of Science and Technology,19 cases of patients with diabetic foot,and 7 cases of orthopaedic patients without diabetes. A total of 26 patients were recruited and were divided into four groups: patients with diabetic foot wound with a large number of bacteria > 1 ×10~6/g tissue for 7 cases of group A,patients with diabetic foot wound with a small number of bacteria < 1 ×10~6a/g tissue for 12 cases of group B,diabetes without wound tissue(from diabetic foot ulcers and skin edge was greater than 2 cm) for6 cases of group C,orthopaedic patients without diabetes for 7 cases of group D. Biopsies were evaluated by both RT-qPCR and Western Blot to assess the levels of MMP-9 and VEGF. Differences between groups with single factor analysis of variance analysis and Scheffe test,comparison between the two groups used q test.Results The relative expression of MMP-9 in group A,B,C,D were 1. 14 ± 0. 12,0. 60 ± 0. 08,0. 39 ±0. 09,0. 13 ± 0. 06,differences between the four groups were in a decreasing trend(F = 43. 92,P < 0. 05).The relative expression levels of VEGF in group A,B,C,D were 1. 01 ± 0. 22,3. 55 ± 0. 97,5. 72 ± 0. 55,6. 92 ± 1. 55,the difference between the four groups showed an ascending tendency(F = 40. 74,P < 0. 05).The results of western Blot test were consistent with the results of RT-PCR. Conclusion The expression of MMP-9 increased in different diabete wound tissue,and the expression of VEGF decreased.
