摘要
目的了解我国2008—2016年16省(市/区)16~25岁异性性行为感染艾滋病病毒(HIV)者中HIV毒株亚型亚簇的分布情况。方法收集调查对象血浆样本,提取HIV核糖核酸并扩增其Pol区片段(PR区,RT区),应用HIV Sequence Database网站和MEG 6.0等软件等进行亚型亚簇分析。结果本研究共分析pol区序列1 779条,HIV基因分型显示:856例男性感染者中CRF01AE亚型346例(40.4%)和CRF07BC亚型298例(34.8%),主要亚簇是CRF07BC的MSM簇(69.5%,207/298);898例女性感染者CRF01AE亚型323例(36.0%)和CRF07BC亚型281例(31.3%),主要亚簇是CRF01AE的C1簇(51.1%,165/323)。男性和女性感染者中亚型亚簇中呈上升趋势的有CRF01AE的C5簇、CRF07BC的MSM簇及CRF5501B(P<0.01)。男性感染者和女性感染者中新型重组或者未知亚型毒株的构成比分别为12.0%和15.3%。结论 16~25岁年龄组男性和女性异性感染者中流行HIV毒株的亚簇不同,源自男男性行为者(MSM)的CRF01AE的C5簇、CRF07BC的MSM簇及CRF5501B毒株在我国小年龄异性性传播感染者中呈上升趋势。
Objective To understand the distribution of HIV strains among 16 to 25 years old heterosexually transmitted infections in 16 provinces(cities)in China from 2008 to 2016.Methods Plasma samples were collected from the subjects,HIV RNA was extracted and the Pol region fragment(PR region,RT region)was amplified,and subtypes and clusters analysis was performed by using software such as HIV Sequence Database and MEG 6.0.Results A total of 1 779 pol sequences were analyzed in this study.HIV genotyping showed that the main subtypes and clusters of male infected persons were CRF01AE(40.4%,346/856)and CRF07BC(34.8%,298/856),and the main cluster were cluster MSM of CRF07BC(69.5%,207/298).The main subtypes and clusters of female infected persons were CRF01AE(36.0%,323/896)and CRF07BC(31.3%,281/896),and the main cluster was cluster 1 of CRF01AE(51.1%,165/323).The cluster 5 of CRF01AE,cluster MSM of CRF07BC and CRF5501 B(P<0.01)were observed,showing an upward trend in male and female infected individuals.The composition ratio of new recombinant or unknown subtype strains in male infected and female infected persons was 12.0%and15.3%,respectively.Conclusion The cluster 5 of CRF01AE derived from MSM population,the cluster MSM of CRF07BC and the CRF5501 Bstrain show an increase in small-age heterosexually transmitted infections in China.
