摘要
目的:探讨新生儿窒息的产科因素及脐动脉血气分析与新生儿窒息的相关性,研究动脉血气分析对新生儿窒息的预测价值。方法:选取2009-012015-01在我院分娩的1 024例新生儿,其中有260例新生儿窒息,所有新生儿均在出生后1min内进行脐动脉血气分析和Apgar评分。根据研究数据分析新生儿窒息的产科因素及1min内脐动脉血气分析、Apgar评分与新生儿窒息的相关性。结果:脐带因素、羊水因素、产程异常、胎位异常、早产、胎盘功能不全等是新生儿窒息的高危因素。新生儿动脉血PCO2与1min Apgar评分呈负相关,而PO2、BE、pH则与Apgar评分呈正相关;Apgar评分≤3的新生儿与Apgar评分>3的新生儿脐动脉血气分析结果比较,差异有统计学意义(P<0.01);Apgar评分≤7与Apgar评分>7的新生儿动脉血气分析结果比较,差异有统计学意义(P<0.01)。新生儿重度窒息、轻度窒息与脐带血PH呈负相关(P<0.01),正常组新生儿与脐带血PH呈正相关(P<0.01)。结论:降低新生儿窒息的发生率需做好产前保健、缩短第二产程、及时处理胎儿宫内窘迫及防治各种妊娠并发症等。脐动脉血气分析作为Apgar评分的有效补充对临床新生儿窒息的预测与诊断有着重要的意义,能够有效提高新生儿窒息的诊断率。
Objective:To explore the correlations and predictive values of obstetric factors and umbilical artery blood gas analysis in neonatal asphyxia.Methods:A total of 260 newborns of neonatal asphyxia from January 2009 to January 2015 in our hospital were selected and processed with umbilical artery blood gas analysis and Apgar score within 1min after birth.Results:There was a negative correlation beween arterial blood PCO2 and 1min Apgar score,while there were a positive correlation among PO2,BE,pH and Apgar score.There appeared significant differences between Apgar score≤3neonatal and Apgar score>3(P<0.01),and Apgar score≤7and Apgar score>7neonatal in umbilical arterial blood gas analysis(P<0.01).Conclusion:Umbilical artery blood gas analysis can be an effective alternative in the prediction and diagnosis of neonatal asphyxia.
