摘要
目的 探讨CyclinD1、p1 6和p5 3在乳腺癌中的表达及意义。 方法 采用免疫组化SP法对 2 3例乳腺增生症、1 1例乳腺不典型增生、5 8例乳腺癌中CyclinD1、p1 6、p5 3、ER、PCNA进行检测。 结果 CyclinD1、p1 6、p5 3的表达在乳腺增生症与不典型增生、乳腺癌间差异均有统计学意义。CyclinD1的表达与乳腺癌淋巴结和 (或 )器官转移状态、3年生存情况、ER的表达、PI分级相关 ,与组织学分级、TNM分期无关 ;p1 6和p5 3的表达与各临床病理特征均相关。p1 6与CyclinD1 ,p1 3的表达呈相关关系 (P <0 .0 1 ) ,CyclinDl与p5 3的表达亦呈相关关系 (P <0 .0 1 )。乳腺癌 ,不典型增生多基因异常表达显著高于乳腺增生症 (P <0 .0 1 ,P <0 .0 5 ) ,多基因异常表达与乳腺癌各临床病理特征均相关 (P <0 .0 1 )。 结论 CyclinDl、p5 3的过表达与p1 6的失表达在乳腺癌的发生、发展过程中可能起着重要作用 ;三者联合检测优于单项检测 ,可提高乳腺癌早期诊断、恶性程度及预后判断的可靠性 。
Objective To study the expressions of Cyclin D1, p16 and p53 in breast carcinoma and their significance. Methods Cyclin D1, p16, p53, ER and PCNA were detected by immunohistochemical technique (SP method) in 23 cases of proliferative breast disease, 11 cases of atypical hyperplasia and 58 cascs of breast carcinoma. Results There was statistically significant difference in expressions of Cyclin D1,p16 and p53 among proliferative breast disease, atypical hyperplasia and breast carcinoma. Cyclin D1 was related to transfer status of lymph node and (or) organ, survival rate of 3 years, expression of ER, grades of proliferative index of breast carcinoma, but not related to histologic grades, TNM classification. Expressions of p16 and p53 were correlated with the clinicopathologic characters respectively. There was a negative correlation among the expressions of Cyclin D1, p53 and p16 (P<0.01), a positive correlation between the expressions of p53 and Cyclin D1 (P<0.01). Abnormal expressions of multiple genes in breast carcinoma and atypical hyperplasia were much higher than that in proliferative breast disease(P<0.01, P<0.05). Abnormal expression of multiple genes in breast carcinoma was correlated with the clinicopathologic characters respectively (P<0.01). Conclusion Over expressions of Cyclin D1, p53 and nagetive expression of p16 may play an important role in the genesis and developments of breast carcinoma; Associated detection of them is better than detection of single one, it can enhance the reliability of early diagnosis, estimating malignant degree and prognosis of breast carcinoma.
