Cardioprotective effect of analgesia during acute myocardial infarction in rabbits

摘要 目的 探讨家兔心肌梗死急性期痛觉干预对心肌的保护作用。方法 2.5~3.0kg健康家兔21只,随机分为三组,每组7只。对照组:单纯结扎冠状动脉左对角支;曲马多组:于结扎冠脉前10min静脉注射曲马多4 mg/kg,并于结扎后3h等量补充;吗啡组:与曲马多组同时点静脉注射吗啡0.4 mg/kg。各组均行左股动脉穿刺监测平均动脉压(MAP),并分别于结扎前(基础值)、结扎后3h、6 h抽取颈外静脉血,测肌钙蛋白(TnI)及去甲肾上腺素(NE)的含量。结果 MAP:结扎冠脉后1、2、3、6h对照组较基础值明显降低(P<0.01),曲马多组与基础值差异无显著性,吗啡组MAP的变化同曲马多组;曲马多组在结扎冠脉后1、2、3hMAP高于对照组(P<0.05),吗啡组在结扎后各时点,MAP与对照组相比差异无显著性。TnI:结扎冠脉前基础TnI值组间差异无显著性。结扎后3h和6h曲马多组和吗啡组均值低于对照组(P<0.05)。曲马多组和吗啡组之间各时点差异不明显。NE:组间差异不明显。结论 使用曲马多和吗啡实施痛党干预对心肌梗死急性期的家兔心肌具有一定的保护作用。 Objective To determine if analgesia during acute myocardial infarction is protective to the ischemic myocardium.Methods Twenty-one healthy rabbits of either sex, weighing 2.5-3.0 kg were randomly divided into 3 groups of seven animals : (1) control group in which the circumflex branch of left coronary artery was occluded (CAO); (2) tramadol group received tramadol 4 mg.kg-1 iv 10 min before and 3h after CAO; (3) morphine group received morphine 0.4 mg.kg-1 iv 10 min before and 3 h after CAO. MAP was recorded before (baseline) and 1, 2, 3, 6 h after CAO. Blood samples were obtained from external jugular vein before (baseline) and 3 h, 6 h after CAO for determination of serum levels of troponin (TnI) and noradrenaline (NE). Results (1) M AP decreased significantly at 1, 2, 3, 6 h after CAO as compared with the baseline in control group ( P < 0.01);while in tramadol and morphine group there was no significant change in MAP after CAO compared with the baseline. (2) Serum Tnl concentrations were significantly lower after CAO in tramadol and morphine group than those in control group. (3) There was no significant difference in serum NE levels among the 3 groups. Conclusion Analgesia during acute myocardial infarction has a certain cardioprotective effect in rabbit.
