摘要
目的探讨综合性治疗进行性半侧颜面萎缩的临床疗效。方法自2010年1月至2017年12月,针对12例不同程度进行性半侧颜面萎缩患者,采用自体脂肪移植、LefortⅠ型截骨、双侧下颌升支矢状劈开截骨术、下颌骨牵引成骨、扩张头皮瓣移植等多种方式结合的治疗方法,并观察患者面部不对称的改善效果。结果根据Guerrerosantos临床分型个性化评估12例患者病情,经以软组织重塑的自体脂肪移植结合其他手术方式治疗后,患者局部面部凹陷明显改善,面部不对称性明显矫正。术后随访6~12个月,疗效满意。结论根据病情分型采用骨骼轮廓重建联合软组织重塑的综合策略治疗进行性半侧颜面萎缩,临床效果显著,是较为合理的治疗方式。
Objective To explore the clinical efficacy of the systematic treatment of progressive hemifacial atrophy.Methods From January 2010 to December 2017,12 patients with progressive hemifacial atrophy were treated by autologous fat transplantation,Le Fort I osteotomy,sagittal split ramus osteotomy(SSRO),mandibular distraction osteogenesis(DO),adjacent scalp expanded flap transplantation,and other methods.The postoperative improvement of the symmetry of the face was observed and evaluated.Results Personalized assessment of 12 patients was conducted according to the Guerrerosantos classification.All patients underwent autologous fat transplantation combined with other operations for soft tissue reconstruction.The facial appearance was obviously improved,and the asymmetry of the faces was obviously corrected.All patients were followed-up for 6 months,and the curative effect was satisfactory.Conclusion The combined application of bone framework reconstruction,occlusal function reconstruction,and soft tissue augmentation is a feasible systematic method based on the different classifications of progressive hemifacial atrophy.The clinical result was significant.
