摘要
目的观察补阳还五汤联合奥扎格雷钠治疗心源性脑栓塞的临床疗效。方法选取2014年2月—2015年11月于扶沟县人民医院住院治疗的心源性脑栓塞患者97例,随机分为对照组(n=48)与观察组(n=49)。对照组给予常规西药(包括低分子肝素钙、复方丹参注射液、甘露醇、右旋糖酐、抗血小板聚集等)治疗,观察组在对照组基础上加用补阳还五汤联合奥扎雷钠治疗。比较两组患者临床疗效、血液流变学指标、日常生活能力量表(BI)评分及美国国立卫生研究院卒中量表(NIHSS)评分情况。结果观察组总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前两组患者全血高、低切黏度,血浆黏度、三酰甘油、胆固醇及纤维蛋白原水平比较,差异无统计学意义(P>0.05);治疗后观察组血液黏度、血浆黏度、三酰甘油、胆固醇及纤维蛋白原水平低于对照组,差异有统计学意义(P<0.05)。治疗前两组NIHSS评分及BI评分比较,差异无统计学意义(P>0.05);治疗后观察组NIHSS评分低于对照组,BI评分高于对照组,差异有统计学意义(P<0.05)。结论补阳还五汤联合奥扎格雷钠治疗心源性脑栓塞的疗效确切,可改善患者血液流变学指标及脑组织血液循环,并提高患者日常生活能力。
Objective To investigate the clinical effect of buyang huanwu decoction combined with sodium ozagrel in the treatment of cardioembolism. Methods A total of 97 patients with cardioembolism were treated in the People' s Hospital of Fugou County from February 2014 to November 2015,which were randomly diviced into control group( n = 48) and observation group( n = 49). The control group were treated with conventional medical( including low molecular heparin calcium,danshen injection,mannitol,dextranum,anti- platelet aggregation etc),the observation group were treated with buyang huanwu decoction combined with sodium ozagrel on the basis of control group,the clinical effect,hemorheology index,Barthel( BI)score and national institutes of health stroke scale( NIHSS) score of two groups were compared. Results The total effective rate of observation group was higher than that of control group( P < 0. 05). Before treatment,there was no statistically significant differences in high blood viscosity,low blood viscosity,plasma viscosity,triacylglycerol,cholesterol and fibrinogen between the two groups( P > 0. 05); after treatment high blood viscosity,low blood viscosity,plasma viscosity,triacylglycerol,cholesterol and fibrinogen of the treatment group were lower than the control group( P < 0. 05). Before treatment,there was no statistically significant differences in NIHSS and BI scores between the two groups( P > 0. 05); after treatment NIHSS score of the treatment group was lower than the control group,BI score was higher than the control group( P < 0. 05). Conclusion Buyang huanwu decoction combined with sodium ozagrel have significant clinical effect in treating cardioembolism,can improve hemorheology index and blood circulation of brain tissue,enhance activity of daily living.
