心源性脑栓塞急性期体外循环脑损伤机制及脑保护的研究进展 被引量:10

摘要 心源性脑栓塞( cardiogenic embolism CE )是指由来自心脏的脱落栓子阻塞脑供血动脉,从而引起相应供血区脑组织的缺血性坏死,占整个缺血性卒中的20%[1],其高危因素包括左心房血栓、二尖瓣狭窄、左心房黏液瘤、感染性心内膜炎及冠心病等。 CE后应尽快清除心内的附壁血栓、瓣膜上的赘生物或矫正器质性病变,以防止CE后的短期内再次发生脑栓塞或心力衰竭加重,导致患者面临更加危险的境地[2]。而在体外循环( cardiopulmonary bypass , CPB )下进行的心脏手术有可能导致脑栓塞后脑细胞的进一步损害。本文将对CE急性期这一特定的病理条件下施行CPB的脑损伤机制及脑保护措施的研究进展进行综述分析。
