摘要
目的 :探讨在胃阴虚型慢性胃炎患者中自拟石斛益胃汤辨证加味的疗效。方法 :选取医院门诊2016年1月—2018年1月收治的120例胃阴虚型慢性胃炎患者,借助随机数表分为常规组和加味组,常规组60例患者实施常规西医治疗,加味组60例患者实施常规西医与自拟石斛益胃汤辨证加味治疗。对比治疗前后主症、次症中医证候积分及总积分变化,治疗前后肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白介素-32(IL-32)变化,临床疗效,不良反应发生率;2组均随访6个月,对比复发率。结果 :治疗后2组主症、次症中医证候积分及总积分均显著降低(P<0.05),加味组均低于常规组(P<0.05);2组治疗后血清TNF-α、hs-CRP、IL-32水平均降低(P<0.05),加味组血清TNF-α和hs-CRP水平均低于常规组(P<0.05);加味组临床疗效分布与常规组有显著差异(P<0.05),前者总有效率明显高于后者(P<0.05);加味组和常规组不良反应发生率相近(P>0.05),且前者复发率远低于后者(P<0.05)。结论 :对胃阴虚型慢性胃炎采用自拟石斛益胃汤辨证加味治疗能够显著减轻症状,控制炎症反应,增强疗效,还可降低复发率,且并不会明显增多不良反应。
Objective:To explore the treatment effect of self-made Shihu Yiwei Decoction with syndrome differentiation and modification on patients with stomach Yin deficiency type chronic gastritis.Methods:120 patients with stomach Yin deficiency type chronic gastritis were selected from January 2016 to January 2018,who were divided into routine group and modified group by random number table.The routine group with 60 patients was given routine Western medicine treatment.The modified group with 60 patients was reated with conventional Western medicine and self-made Shihu Yiwei Decoction with syndrome differentiation and modification.The changes of main and secondary TCM syndromes integral and total integral before and after treatment were compared,as well as the changes of tumor necrosis factor-α(TNF-α),hypersensitive C-reactive protein(hs-CRP)and interleukin-32(IL-32),the clinical efficacy and the incidence of adverse reactions.The 2 groups were followed up for 6 months,and the recurrence rates were compared.Results:After treatment,the main and secondary TCM syndromes integral and total integral in the 2 groups were significantly decreased(P<0.05),which in the modified group were lower than those in the routine group(P<0.05).After treatment,the levels of serum TNF-α,hs-CRP and IL-32 in the two groups were all decreased(P<0.05),while those in the modified group were lower than those in the routine group(P<0.05).There was a significant difference in clinical efficacy between the modified group and the routine group(P<0.05).The total effective rate of the former was significantly higher than that of the latter(P<0.05).The incidence of adverse reactions was similar between the modified group and the routine group(P>0.05),and the recurrence rate of the former was much lower than that of the latter(P<0.05).Conclusion:Selfmade Shihu Yiwei Decoction with syndrome differentiation and modification treating patients with stomach Yin deficiency type chronic gastritis could significantly relieve symptoms,control inflammation,enhance
