摘要
目的:探讨头电针改善脑小血管病患者(CSVD)步态及平衡功能障碍的临床效果。方法:将符合纳入标准的CSVD患者随机分为对照组和治疗组。两组患者均给予对症支持治疗和体针治疗;治疗组在此基础上加用头电针。治疗前后,采用步态分析系统评价患者;采用Berg平衡量表和Tinetti平衡与步态量表中平衡能力测试部分评价患者的平衡功能。采用起立行走计时测验(TU>)和站起测验(CRT)评价跌倒风险。结果:治疗后,两组患者的步长、步速和步频均较本组治疗前提高(P <0. 05,P <0. 01);两组患者的支撑相百分比较治疗前有所降低;而摆动相百分比较治疗前显著提高(均P <0. 05)。两组患者的足廓清、足跟着地角度和脚尖离地角度均较本组治疗前有增加(P <0. 05,P <0. 01)。治疗组对上述指标的改善作用明显优于对照组(P <0. 05,P <0. 01)。治疗后,两组患者的BBS和Tinetti评分均较治疗前明显提高(P <0. 05,P <0. 01);完成TU>和CRT测验所需时间均较本组治疗前降低;且治疗组对上述指标的改善作用优于对照组(P <0. 05)。结论:体针辅以头电针治疗CSVD步态障碍效果良好,并有维持患者躯体平衡、降低跌倒风险的作用,值得在临床进一步推广应用。
Objective:In this study,we investigated whether electro-scalp acupuncture improves gait and balance dysfunction in patients with cerebral small vascular disease(CSVD).Methods:Patients with CSVD who met the inclusion criteria were randomly assigned to the control and treatment groups.Patients in the two groups were given body acupuncture treatment.Besides,the treatment group was given electro-scalp acupuncture treatment furtherly.Before and after treatment,gait of patients were evaluated by the gait analysis system and the balance function of patients were evaluated using the Berg Balance Scale and Tinetti Performance Oriented Mobility Assessment.The fall risk was assessed using the TU>and CRT test.Results:After treatment,the stride length,walking speed and cadence of the two groups were higher than those before treatment(P<0.05,P<0.01).The percentage of stance phase of the two groups was lower than that before treatment whereas the percentage of swing phase was higher than that before treatment(all P<0.05).The clearance of the foot,the heel strike angle and the toe-off angle of the two groups were increased compared with before treatment in the same group(P<0.05,P<0.01).The improvement effect of the treatment group on the above indicators were significantly better than those of the control group(P<0.05,P<0.01).After treatment,the BBS and Tinetti scores of the two groups were significantly higher than those before treatment(P<0.05,P<0.01).The time required to complete the TU>and CRT tests were lower than those before treatment in the same group.The improvement effect for the above indexes was better for the treatment group when compared with that of the control group(P<0.05).Conclusion:Body acupuncture combined with electro-scalp acupuncture treatment could improve gait disturbances,maintain body balance and reduce the falling risk of CSVD patients.It is worthy of further promotion and application in the clinical practice.
