摘要
脑小血管病(cerebral small vessel disease,CSVD)是临床常见的一种缓慢进行的脑部血管疾病。随着病情进展,患者出现进行性认知障碍、痴呆、中风症状、步态障碍、括约肌功能障碍和精神障碍等,严重影响患者生活质量。中医学并无其对应病名,基于其临床表现的不同,可将其归属于中医学“痴呆”“中风”“郁证”的范畴。近年来该病的发病率呈逐渐上升趋势,随着国内外医疗水平的不断发展,对CSVD的发病机制及治疗有了更深入的认识。故该文通过查阅近年来国内外相关文献,就CSVD的西医发病机制、中医病因病机及中西医治疗等问题归纳论述,旨在为今后CSVD的研究以及临床治疗提供依据和参考,并对中西医研究现状与未来发展方向进行思考与展望。
Cerebral small vessel disease(CSVD)is a common clinical slow-moving cerebral vascular disease.With the progress of the disease,patients have progressive cognitive impairment,dementia,stroke symptoms,gait disorders,sphincter dysfunction and mental disorders,which seriously affect the quality of life of patients.There is no corresponding disease name in traditional Chinese medicine.Based on the different clinical manifestations,it can be classified into the categories of“dementia”“stroke”and“depression syndrome”in traditional Chinese medicine.In recent years,the incidence rate of this disease has gradually increased.With the continuous development of medical care at home and abroad,we have a deeper understanding of the pathogenesis and treatment of CSVD.Therefore,by consulting the relevant literature at home and abroad in recent years,this paper summarizes and discusses the pathogenesis of CSVD in western medicine,etiology and pathogenesis of traditional Chinese medicine and treatment of traditional Chinese and Western medicine,in order to provide basis and reference for the research and clinical treatment of CSVD in the future,and think and prospect the research status and future development direction of traditional Chinese and Western medicine.
