摘要
目的探讨甲胎蛋白异质体(AFP-L3)、凝血酶原时间(PT)、活化部分凝血酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(Fg)对于早期诊断原发性肝癌(PHC)的临床价值。方法选取经病理学检查确诊PHC患者100例(PHC组)、肝硬化患者100例(肝硬化组)、健康体检对象100例(健康组),收集时间2016年1月-2017年9月,检测三组的AFP-L3、PT、APTT、TT、Fg水平并进行比较,采用受试者工作曲线分析五项指标单独及联合应用诊断PHC的价值。结果 PHC组的AFPL3、PT、APTT、TT检测值显著的高于对照组(P<0.05);PHC组的Fg检测值显著的低于对照组(P<0.05);PHC组AFP-L3、Fg显著的高于肝硬化组(P<0.05);肝硬化组的PT、APTT、TT显著的高于对照组(P<0.05),肝硬化组的Fg低于PHC组(P<0.05);血清AFP-L3、PT、APTT、TT、Fg联合应用诊断PHC的灵敏度为94.19%、特异度为90.73%、漏诊率为5.18%、误诊率为9.27%、AUC值为0.918。结论血清AFP-L3、PT、APTT、TT、Fg联合应用对于早期诊断PHC具有一定的临床价值。
Objective To investigate the clinical value of alpha fetoprotein heterogeneity(AFP-L3),prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and fibrinogen(Fg)in the early diagnosis of primary liver cancer(PHC).Methods The histologic diagnosis of 100 PHC patients(PHC group),100 patients with liver cirrhosis(cirrhosis group)and 100 cases of healthy subjects(healthy group)between January 2016 to September 2017 were collected,compare the three groups of AFP-L3,PT,APTT,TT level using receiver operating curve analysis of the five indicators used alone and combined diagnostic value of PHC.Results PHC group of AFP-L3,PT,APTT,TT,Fg detection was significantly higher than that of control group(P<0.05);Fg detection of PHC group was significantly lower than that of the control group(P<0.05);PHC AFP-L3 group,Fg was significantly higher than that of cirrhosis group(P<0.05);liver cirrhosis group PT,APTT,TT was significantly higher than that of control group(P<0.05),liver cirrhosis group Fg than group PHC(P<0.05);the sensitivity of serum AFP-L3,PT,APTT,TT and Fg in the diagnosis of PHC was 94.19%,the specificity was 90.73%,the misdiagnosis rate was 5.18%,the misdiagnosis rate was 9.27%,AUC value is 0.918.Conclusion The combined application of serum AFP-L3,PT,APTT,TT and Fg is of certain clinical value for early diagnosis of PHC.
