摘要
目的探讨血清甲胎蛋白在原发性肝癌检验中的价值。方法选择2018年1月—2021年5月至该院接受检查者350人的血清标本,其中原发性肝癌患者103例(原发性肝癌组)、肝硬化91例(肝硬化组)、慢性乙型肝炎68例(慢性乙型肝炎组)、健康者88名(对照组)。以全自动免疫分析仪检测4组血清标本的甲胎蛋白水平并比较。经Spearman秩相关分析方式分析血清甲胎蛋白水平与原发性肝癌分期以及肿瘤直径的相关性,比较治疗前后血清甲胎蛋白水平变化。结果4组血清标本的血清甲胎蛋白水平对比,差异有统计学意义(P<0.05),其中原发性肝癌组的血清甲胎蛋白水平最高,对照组血清甲胎蛋白水平最低,差异有统计学意义(P<0.05)。原发性肝癌患者(Ⅰ期、Ⅱ期、Ⅲ期)治疗后的血清甲胎蛋白水平明显低于治疗前,差异有统计学意义(P<0.05)。结论血清甲胎蛋白应用于原发性肝癌检验中具有较高的价值,其水平变化与肿瘤大小以及TNM分期有关,可用于原发性肝癌的疗效检测,对此类疾病的诊治有积极意义。
Objective To explore the value of serum alpha-fetoprotein in the detection of primary liver cancer.Methods Serum samples of 350 people who were examined in the hospital from January 2018 to May 2021 were selected.Among them,103 patients with primary liver cancer(primary liver cancer group),91 patients with liver cirrhosis(cirrhosis group),68 patients with chronic hepatitis B(chronic hepatitis B group),and 88 healthy patients(control group).The levels of alpha-fetoprotein in the four groups of serum samples were detected by an automatic immunoassay analyzer and compared.Spearman rank correlation analysis method was used to analyze the correlation between serum alpha-fetoprotein level and primary liver cancer stage and tumor diameter.Compared the changes of serum alpha-fetoprotein levels before and after treatment.Results The serum alpha-fetoprotein levels of the four groups of serum samples were compared,the difference was statistically significant(P<0.05).The primary liver cancer group had the highest serum alpha-fetoprotein level,and the control group had the lowest serum alpha-fetoprotein level,the difference was statistically significant(P<0.05).The serum alpha-fetoprotein level of patients with primary liver cancer(stageⅠ,stageⅡ,and stageⅢ)after treatment was significantly lower than before treatment,the difference was statistically significant(P<0.05).Conclusion Serum alpha-fetoprotein is of high value in the detection of primary liver cancer,and its level changes are related to tumor size and TNM staging.It can be used to detect the curative effect of primary liver cancer and has positive significance for the diagnosis and treatment of such diseases.
