摘要
醒脑开窍法、阴阳配穴法、分期配穴法为临床针刺治疗中风后肢体痉挛常用的治疗方法;传统针刺方法、经筋理论、现代针刺方法、不同病因病机、特殊针法及其他疗法等为针刺治疗中风后肢体痉挛常用的配穴原则作指导,其中传统针刺方法指导的取穴原则包括恢刺、关刺、巨刺等对于患肢痉挛的改善有重要的意义。但是配穴原则及针刺方法的不尽相同,使治疗时针刺的有效刺激量无明确的量化标准,可重复性差。此外,目前的评价标准多以康复评定量表为主,评价疗效无统一标准,更是缺乏独立的中医疗效评定方案,针对纳入病例的患者的针刺治疗方法,大多是结合康复治疗为主,无法突出针刺的主要疗效,且临床实验样本较小,缺少多中心大样本临床观察。
Brain-awakening and resuscitation method,yin-yang point matching method,and point matching method by stages are commonly used in the treatment of limb spasm after apoplexy by clinical acupuncture.Traditional acupuncture methods,meridian tendon theory,modern acupuncture methods,different etiology and pathogenesis,special needling and other therapies are commonly used to guide the point matching principles in the treatment of limb spasm after apoplexy.Among them,the point selection principles guided by traditional acupuncture methods,including lateral needling,joint needling,and contralateral channel needling,etc.are of great significance for the improvement of limb spasm.However,the principle of point selection and the method of acupuncture are different,so that there is no clear quantitative standard for the effective stimulation of acupuncture in the treatment,and the repeatability is poor.In addition,the current evaluation criteria are mostly rehabilitation assessment scale.There is no unified standard for evaluation of curative effect,and there is no independent evaluation program of curative effect of TCM.Acupuncture treatment methods for patients included in the medical cases are mostly combined with rehabilitation treatment,which cannot highlight the main effect of acupuncture,and clinical experimental samples are small,and there is a lack of multi-center large sample clinical observation.
