三阴性乳腺癌中E-钙黏蛋白的表达及其临床意义 被引量:3

摘要 目的探讨三阴性乳腺癌组织中的E-钙黏蛋白(E-cadherin,E-cad)的表达及其与临床病理指标和预后的关系。方法 2002年1月—2008年10月在汕头大学医学院附属肿瘤医院接受治疗的女性三阴性乳腺癌组织35例,采用免疫组织化学GTvision两步法检测样本中E-cad的表达,采用卡方检验分析其与临床病理特征的关系,应用Kaplan-Meier法和Cox比例风险回归模型分析E-cad表达对预后的影响。结果 E-cad主要定位在细胞膜,35例三阴性乳腺癌组织中E-cad的低表达率为51.4%(18/35),其表达与临床肿瘤分期(P=0.009)和淋巴结转移有关(P=0.044)。E-cad低表达和高表达患者总体生存曲线差异有统计学意义(Log-rank检验,P=0.014)。Cox比例风险回归模型分析示E-cad是三阴性乳腺癌患者独立的预后因素(风险比0.241,95%置信区间0.072~0.814,P=0.022)。结论 E-cad低表达或缺失更容易引起三阴性乳腺癌的浸润转移,在三阴性乳腺癌的发展过程中起着重要作用,可以作为三阴性乳腺癌患者预后的一个重要的独立预测因素。 Objective To study the protein expression of E-cadherin( E-cad) to understand its relationship with clinicopathological parameters and its influence on the prognosis of human triple negative breast carcinoma( TNBC). Methods We collected tumor tissues from 35 female patients with TNBC treated at the Cancer Hospital of Shantou University Medical College from January 2002 to October 2008. Immunohistochemistry was performed to detect the protein expression of E-cad in breast tumors,and the chi-square test was used to study the relationship between the E-cad expression and clinicopathological parameters. The Kaplan-Meier method and Cox regression model were conducted to study the influence of E-cad on the prognosis of TNBC. Results E-cad protein was expressed primarily in the cytomembrane. The low expression rate of E-cad in 35 TNBC tumors was51. 4%( 18 / 35). The chi-square test revealed that the E-cad expression was related to TNM stage( P = 0. 009) and lymph node metastasis( P = 0. 044). The survival curves between the low protein isoaspartyl methyltransferase expression group and the high protein isoaspartyl methyltransferase expression group were significantly different( Log-rank P = 0. 014). Cox regression model demonstrated that E-cad( hazard ratio 0. 241,95% confidence interval 0. 072 — 0. 814,P = 0. 022) was the independent prognostic factors for TNBC. Conclusion E-cad plays an important role in the progression of breast cancer and could be a novel prognostic factor for TNBC.
