摘要
目的探讨胰岛素泵与血液透析联合治疗糖尿病酮症酸中毒(diabetic ketoacidosis,DKA)的疗效及安全性。方法选取2019年1月—2024年6月江苏大学附属第一人民医院收治的80例DKA患者为研究对象,根据不同的治疗方法分为对照组(40例,接受胰岛素泵治疗)和观察组(40例,接受胰岛素泵与血液透析联合治疗)。对比两组血糖水平、血气水平、症状积分、并发症发生率。结果治疗后,观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、多尿烦渴评分、恶心呕吐评分、呼吸深快评分、头痛嗜睡评分、并发症发生率均低于对照组,差异有统计学意义(P均<0.05)。观察组动脉血pH值、HCO-(3)^(+)水平、二氧化碳结合力均高于对照组,差异有统计学意义(P均<0.05)。结论胰岛素泵与血液透析联合治疗DKA相较于单纯胰岛素泵治疗,能更有效地降低血糖、改善血气水平、减轻症状,降低并发症发生率,显示出更好的疗效和安全性。
Objective To investigate the efficacy and safety of insulin pump combined with hemodialysis in the treatment of diabetic ketoacidosis(DKA).Methods A total of 80 patients with DKA admitted to the First People's Hospital Affiliated to Jiangsu University from January 2019 to June 2024 were selected as the research objects.According to different treatment methods,they were divided into control group(40 cases,treated with insulin pump)and observation group(40 cases,treated with insulin pump combined with hemodialysis).The blood glucose level,blood gas level,symptom score and complication rate were compared between the two groups.Results After treatment,the fasting blood glucose,2-hour postprandial blood glucose,glycated hemoglobin A1c,polyuria and thirst score,nausea and vomiting score,deep and fast breathing score,headache and drowsiness score and complication rate in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).The arterial blood pH value,HCO-(3)^(+)level and carbon dioxide binding capacity in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion Compared with simple insulin pump treatment,the combination of insulin pump and hemodialysis in the treatment of DKA can more effectively reduce blood glucose,improve blood gas level,relieve symptoms,and reduce the incidence of complications,showing better efficacy and safety.
