胰岛素静脉滴注与持续皮下注射胰岛素泵治疗重症糖尿病酮症酸中毒的临床研究

摘要 目的 分析重症糖尿病酮症酸中毒(diabetic ketoacidosis, DKA)患者实施胰岛素静脉滴注与持续皮下胰岛素泵注射治疗的效果。方法 选取2021年6月—2024年3月福建省泉州市惠安县总医院重症医学科收治的60例重症DKA患者为研究对象,按治疗方法不同分为对照组、观察组,各30例。对照组采用胰岛素静脉滴注治疗,观察组经胰岛素泵给予持续性胰岛素输注。比较两组临床指标、血糖指标、临床疗效、不良反应。结果 观察组胰岛素使用剂量、pH值恢复时间均少于对照组,谷胱甘肽过氧化物酶、超氧化物歧化酶、总抗氧化能力均高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组空腹血糖、餐后2 h血糖均低于对照组,差异有统计学意义(P均<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论 持续皮下胰岛素泵注射治疗重症DKA患者,可有效提升其临床疗效,为患者安全提供保障。 Objective To analyze the effects of intravenous insulin infusion and continuous subcutaneous insulin pump for patients with severe diabetic ketoacidosis(DKA).Methods A total of 60 patients with severe DKA admitted to the Department of Intensive Care Medicine of Hui'an County General Hospital of Quanzhou City,Fujian Province from June 2021 to March 2024 were selected as the study objects,and were divided into control group and observation group according to different treatment methods,with 30 cases in each group.The control group was treated with intra-venous insulin infusion,and the observation group was given continuous insulin infusion via insulin pump.The clini-cal indexes,blood glucose indexes,clinical efficacy and adverse reactions were compared between the two groups.Results The insulin dose and pH recovery time in the observation group were less than those in the control group,and the glutathione peroxidase,superoxide dismutase and total antioxidant capacity were higher than those in the control group,the differences were statistically significant(all P<0.05).After treatment,the fasting blood glucose and 2-hour postprandial blood glucose in the observation group were lower than those in the control group,and the differences were statistically significant(both P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the obser-vation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Continuous subcutaneous insulin pump injection in the treatment of severe DKA patients can effectively improve its clinical efficacy and provide a guarantee for patient safety.
