摘要
目的:评价胰岛素泵治疗糖尿病酮症酸中毒的临床疗效。方法:选择2018年5月~2020年7月来本院就诊的糖尿病酮症酸中毒患者78例为研究对象,按照不同治疗方法分成两组,对照组接受胰岛素持续静脉滴注治疗,观察组接受胰岛素泵治疗,每组各39例,比较两组临床疗效。结果:观察组患者的临床总治愈率(97.44%)显著高于对照组(82.05%),其并发症发生率(5.13%)比对照组(20.51%)低,统计学差异显著,P<0.05;两组治疗前的血糖水平差异无意义,治疗后空腹血糖和餐后2h血糖水平均显著下降,前后对比差异,P<0.05,治疗后组间血糖水平差异无意义,P>0.05;观察组患者的胰岛素总剂量(0.45±0.06)比对照组(0.68±0.07)少,其血糖达标时间、血酮体转阴时间、尿酮体转阴时间均短于对照组,差异有统计学意义,P<0.05。结论:临床糖尿病酮症酸中毒采取胰岛素泵治疗的血糖控制和酸中毒症状改善效果显著,此方法安全、方便。
Objective:To evaluate the clinical efficacy of insulin pump in the treatment of diabetic ketoacidosis.Methods:78 patients with diabetic ketoacidosis who came to our hospital from May 2018 to July 2020 were selected as the research subjects for investigation.They were divided into two groups according to different treatment methods.The control group received continuous intravenous infusion of insulin for observation.The two groups received insulin pump treatment,39 cases in each group,and the clinical efficacy of the two groups were compared.Results:The total clinical cure rate(97.44%)of the observation group was significantly higher than that of the control group(82.05%),and the complication rate(5.13%)was lower than that of the control group(20.51%).The statistical difference was significant,P<0.05;There was no significant difference in blood glucose levels between the two groups before treatment.Fasting blood glucose and 2h postprandial blood glucose levels decreased significantly after treatment.The difference between before and after treatment was P<0.05,and the difference in blood glucose levels between groups after treatment was meaningless(P>0.05);The total insulin dose of the observation group patients(0.45±0.06)was lower than that of the control group(0.68±0.07),and their blood glucose target time,blood ketone body negative time,and urine ketone body negative time were all shorter than those of the control group,with statistical significance(P<0.05).Conclusion:Insulin pump treatment of clinical diabetic ketoacidosis has a significant effect on blood sugar control and acidosis symptoms improvement.This method is safe and convenient.
