肿瘤内磁敏感信号评分鉴别低级别和高级别脑胶质瘤的Meta分析

Intratumoral Susceptibility Signal Scoring for Differentiating Between Low-Grade and High-Grade Brain Gliomas:a Meta-Analysis

摘要 目的采用Meta分析评价磁敏感加权成像(SWI)肿瘤内磁敏感信号(ITSS)评分在术前对低级别和高级别脑胶质瘤的鉴别诊断。方法PubMed、Embase、Web of Science、中国知网、万方、维普数据库自建库至2024年1月公开发表的有关SWI对脑胶质瘤分级诊断的中英文文献,提取相关资料及参数,运用RevMan 5.4、Meta-Disc 1.4及Stata 12软件进行Meta分析。结果共纳入10项研究,包括616例患者。Meta分析结果显示:SWI ITSS评分对高、低级别胶质瘤鉴别诊断的合并敏感度为0.80(95%CI:0.75~0.84);合并特异度为0.83(95%CI:0.78~0.87);合并阳性似然比为4.63(95%CI:3.50~6.12);合并阴性似然比为0.25(95%CI:0.20~0.31);合并诊断比值比为18.21(95%CI:12.20~27.19);受试者工作特征曲线(ROC)曲线下面积(AUC)为0.88。结论SWI ITSS评分对鉴别术前低级别、高级别胶质瘤的诊断价值较高。 Objective To evaluate the differential diagnosis of low-grade and high-grade gliomas by magnetic sensitivity weighted imaging(SWI)intratumoral susceptibility signals(ITSS)score before surgery by meta-analysis.Methods PubMed,Embase,Web of science,CNKI,Wanfang and VIP database were self-established to extract relevant data and parameters from Chinese and English literature on the classification of glioma by SWI published in January 2024.Meta-analysis was performed using RevMan 5.4,Meta-Disc 1.4 and Stata 12 software.Results A total of 10 studies were included,including 616 patients.The results of meta-analysis showed that the Sen combination of SWI ITSS score in the differential diagnosis of high and low grade glioma was 0.80(95%CI=0.75-0.84).Spe combined=0.83(95%CI=0.78-0.87);+LR combined=4.63(95%CI=3.50-6.12);-LR merge=0.25(95%CI=0.20-0.31);Combined DOR=18.21(95%CI=12.20-27.19);Area under SROC curve(AUC=0.88).Conclusion SWI ITSS score is of high diagnostic value in differentiating preoperative low-grade glioma from high-grade glioma.
