摘要
目的:探讨大脑中动脉(MCA)区急性脑梗死的临床特点,根据梗死灶大小比较分析其影响因素和高分辨MRI的特点,描述其动脉血管特点和斑块特点,并阐明其作用机制。方法:收集MCA区急性脑梗死患者80例,行T_(1)WI、T_(2)WI、DWI、FLAIR、MRA和SWI扫描,依据DWI图像上病灶大小,分为大梗死灶组(最大径≥1.5 cm)和小梗死灶组(最大径<1.5 cm)。比较2组患者一般临床资料、实验室检查结果及T_(2)WI、FLAIR、SWI图像特征。结果:80例中,大梗死灶组45例(56.25%),小梗死灶组35例(43.75%)。大梗死灶组入院美国国立卫生研究院卒中量表(NHISS)评分高于小梗死灶组,住院天数长于小梗死灶组,组间比较差异均有统计学意义(均P<0.05);余一般资料组间比较,差异均无统计学意义(均P>0.05)。2组实验室检查结果比较,差异均无统计学意义(均P>0.05)。T_(1)WI示2组病灶部位差异无统计学意义(P>0.05);FLAIR示2组DWMH>2及PVH>2病灶比例差异均无统计学意义(均P>0.05);SWI示2组微出血比例差异无统计学意义(P>0.05);3D-T_(1)WI示大梗死灶组动脉斑块比例显著高于小梗死灶组(P<0.05)。结论:高分辨MRI的3D-T_(1)WI可清楚观察MCA血管情况和动脉斑块情况,且大梗死灶组的斑块大多数位于MCA的开口侧,动脉狭窄程度较高,且管壁呈非正向重构;小梗死灶组的动脉狭窄程度低,且管壁易发生正向重构。
Objective:To explore the clinical characteristics of acute cerebral infarction in middle cerebral artery(MCA)area,compare and analyze its influencing factors and high-resolution MRI characteristics according to the size of the infarct,describe its arterial vascular characteristics and plaque characteristics,and clarify its mechanism.Methods:80 patients with acute cerebral infarction in MCA area were collected.T_(1)WI,T_(2)WI,DWI,FLAIR,MRA and SWI scans were performed with a 3.0 T high-resolution MRI.According to the lesion size on DWI images,80 patients were divided into a large infarction group(maximum diameter≥1.5 cm)and a small infarction group(maximum diameter<1.5 cm).The general clinical data,the laboratory examination results and the image features of T_(2)WI,FLAIR and SWI sequence between the two groups were compared.Results:Among 80 patients with acute cerebral infarction,there were 45 cases(56.25%)in the large infarction group and 35 cases(43.75%)in the small infarction group.The admission NHISS score of the large infarction group was higher than that of the small infarction group,and the hospitalization days of the large infarction group were longer than that of the small infarct group,and the differences between the two groups were statistically significant(both P<0.05),while the differences of the other general clinical data were not statistically significant(all P>0.05).There were no significant differences in the results of laboratory tests between the two groups(all P>0.05).T_(1)WI sequence scanning showed no statistical difference in the distribution of lesions between the two groups(P>0.05),FLAIR sequence showed no statistical differences in proportions of DWMH>2 or PVH>2(both P>0.05),SWI sequence showed no statistical difference in number of micro-bleeding cases(P>0.05),while 3D-T_(1)WI sequence showed that the number of arterial plaques in the large infarction group was significantly higher than that in the small infarction group(P<0.05).Conclusions:The blood vessels and plaques of MCA can be
