摘要
目的分析老年冠心病(coronary artery heart disease,CHD)患者采用阿司匹林与氯吡格雷联合用药对其治疗效果与不良反应的影响。方法选取2022年3月—2023年3月滕州市中医医院心血管内科收治的94例老年CHD患者为对象。采用随机数字表法分为对照组与研究组,各47例。对照组单独给予阿司匹林用药,研究组于对照组基础上联合氯吡格雷给药。治疗后,针对2组治疗效果、血小板聚集率(platelet aggregation rate,PAR)、纤维蛋白原(fibrinogen,FIB)、部分凝血酶原时间(prothrombin time,PT)、心绞痛症状以及不良反应发生率进行对比分析。结果治疗后,研究组总有效率为97.87%,高于对照组的82.98%(P<0.05);研究组PAR为(51.15±4.21)%、FIB为(2.47±0.56)g/L,低于对照组的(58.43±4.67)%、(3.48±0.70)g/L,而PT为(13.34±1.27)s,长于对照组的(10.07±0.92)s(P<0.05);研究组心绞痛发作次数为(2.01±0.82)次/周、持续时间为(2.12±0.95)min/次,优于对照组的(3.08±0.97)次/周、(4.67±1.28)min/次(P<0.05);研究组不良反应总发生率为4.26%,低于对照组的23.40%(P<0.05)。结论老年CHD同时以阿司匹林与氯吡格雷联用治疗可有效提升疾病治疗效果,改善患者的心绞痛症状,不仅能够降低血栓形成风险,还能减少药物不良反应,疗效优异,安全性佳,临床应用价值较高。
Objective To analyze the effects of aspirin combined with clopidogrel on the therapeutic efficacy and adverse reactions of elderly patients with coronary artery heart disease(CHD).Methods A total of 94 elderly CHD patients admitted to the department of cardiovascular medicine,Tengzhou Traditional Chinese Medical Hospital from March 2022 to March 2023 were selected as the subjects.Random number table method was used to divide the control group and the study group,47 cases each.The control group was given aspirin alone,and the study group was given clopidogrel on the basis of the control group.After treatment,the therapeutic effect,platelet aggregation rate(PAR),fibrinogen(FIB),prothrombin time(PT),angina symptoms and incidence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate of the study group was 97.87%,higher than 82.98%of the control group(P<0.05).PAR and FIB of the study group were(51.15±4.21)%and(2.47±0.56)g/L,lower than(58.43±4.67)%and(3.48±0.70)g/L of the control group,while PT was(13.34±1.27)s and longer than(10.07±0.92)s of the control group(P<0.05).The frequency and duration of angina pectoris in the study group were(2.01±0.82)times/week and(2.12±0.95)min/time,which were better than(3.08±0.97)times/week and(4.67±1.28)min/time in the control group(P<0.05).The total incidence of adverse reactions in the study group was 4.26%,which was lower than 23.40%in the control group(P<0.05).Conclusion The combination of aspirin and clopidogrel in the treatment of elderly CHD can effectively improve the therapeutic effect of the disease,improve the symptoms of angina in patients,not only reduce the risk of thrombosis,but also reduce drug adverse reactions.It has excellent efficacy,good safety,and high clinical application value.
