摘要
目的观察中青年冠心病患者ZNF259基因rs964184位点单核苷酸多态性,探讨其与中青年发生冠心病的关系。方法中青年冠心病患者91例为冠心病组,中青年体检健康者183例为对照组,采用倾向性评分对2组进行1:1匹配,比较匹配前、后2组患者年龄、吸烟比率等及匹配后2组血糖、血脂水平等;应用多重SNaPshot技术分析2组ZNF259基因rs964184位点多态性,比较2组ZNF259基因rs964184位点基因型和等位基因分布频率;采用多因素logistic回归分析中青年发生冠心病的影响因素;比较不同基因型冠心病患者血糖、血脂水平等。结果采用倾向性评分成功匹配冠心病组和对照组各80例,匹配前冠心病组吸烟、饮酒比率(56.04%、53.85%)均高于对照组(37.70%、35.52%)(χ^(2)=8.304,P=0.004;χ^(2)=8.402,P=0.004),女性比率(28.57%)低于对照组(42.08%)(χ^(2)=4.725,P=0.030),年龄与对照组比较差异无统计学意义(P>0.05);匹配后冠心病组年龄、性别比例、吸烟及饮酒比率与对照组比较差异均无统计学意义(P>0.05)。匹配后冠心病组总胆固醇[(4.31±1.17)mmol/L]、三酰甘油[(1.47±0.41)mmol/L]、低密度脂蛋白胆固醇[(2.51±0.70)mmol/L]、空腹血糖[(6.62±1.25)mmol/L]、高敏C反应蛋白[(3.21±0.76)mg/L]水平及体质量指数[(26.49±4.12)kg/m^(2)]均高于对照组[(3.59±1.03)mmol/L、(1.21±0.35)mmol/L、(2.18±0.65)mmol/L、(5.97±1.01)mmol/L、(2.94±0.52)mg/L、(24.68±3.93)kg/m^(2)](P<0.05),高密度脂蛋白胆固醇水平[(0.91±0.23)mmol/L]低于对照组[(1.08±0.31)mmol/L](t=3.939,P<0.001),尿酸、总胆红素水平及收缩压、舒张压与对照组比较差异均无统计学意义(P>0.05)。冠心病组ZNF259基因rs964184位点等位基因G分布频率(55.63%)低于对照组(43.75%)(χ^(2)=4.531,P=0.034),基因型分布频率与对照组比较差异无统计学意义(P>0.05)。ZNF259基因rs964184位点等位基因G是中青年发生冠心病的独立危险因素(OR=1.612,95%CI:1.037~2.506,P=0.034)。C
Objective To observe the single nucleotide polymorphism(SNP)of ZNF259 gene rs964184,and to investigate its correlation with coronary heart disease(CHD)in young and middle-aged patients.Methods Ninety-one young and middle-aged CHD patients(CHD group)and 183healthy volunteers(control group)were matched by the propensity score matching method 1∶1.The age and percentage of smoking before and after matching,as well as the plasma glucose and blood lipid levels after matching were compared between two groups.The rs964184polymorphisms of ZNF259 gene were analyzed by SNaPshot technique,and the distribution frequencies of the rs964184genotypes and alleles were compared between two groups.Multivariate logistic regression was used to analyze the influencing factors of CHD in young and middle-aged patients.The plasma glucose and blood lipid levels were compared in CHD patients with different genotypes.Results Eighty patients in each group were matched successfully.Before matching,the percentages of smoking and drinking were higher in CHD group(56.04%,53.85%)than those in control group(37.70%,35.52%)(χ^(2)=8.304,P=0.004;χ^(2)=8.402,P=0.004),the female ratio was lower in CHD group(28.57%)than that in control group(42.08%)(χ^(2)=4.725,P=0.030),and there was no significant difference in the age between two groups(P>0.05).After matching,there were no significant differences in the age,gender ratio,and percentages of smoking and drinking between two groups(P>0.05),the levels of total cholesterol,triacylglycerol,low-density lipoprotein cholesterol,fasting plasma glucose,high-sensitivity C-reactive protein and body mass index were higher in CHD group[(4.31±1.17)mmol/L,(1.47±0.41)mmol/L,(2.51±0.70)mmol/L,(6.62±1.25)mmol/L,(3.21±0.76)mg/L,(26.49±4.12)kg/m^(2)]than those in control group[(3.59±1.03)mmol/L,(1.21±0.35)mmol/L,(2.18±0.65)mmol/L,(5.97±1.01)mmol/L,(2.94±0.52)mg/L,(24.68±3.93)kg/m^(2)](P<0.05),the level of high-density lipoprotein cholesterol was lower in CHD group[(0.91±0.23)mmol/L]than that in control group
