摘要
慢性泄泻是指持续或反复发作超过4周的腹泻症状。西医治疗通常采用标准化的治疗方案,缺乏个性化的考虑,不同患者的体质、病情严重程度等因素都会影响治疗效果,因此标准化的治疗方案可能无法满足所有患者的需求;而中医药在慢性泄泻的治疗中具有个体化治疗、综合调理、副作用小、注重预防和疗效持久等优势。本文针对慢性泄泻的常见辨证分型,探讨了慢性泄泻的诊治以及山前汤的来源和加减应用。山前汤是国医大师张磊教授所创临证八法之燮理法的代表经验方,由生山楂、炒山楂、生车前子、炒车前子等药组成,专为阴阳失调、积湿积热病症所设。山前汤方中药物生熟并用,具有燮理阴阳,消积止泻之功,临证中对于阴阳失调、积湿积热的慢性泄泻具有效如桴鼓的特点,但山前汤临床应用时可不必局限于治疗泄泻,凡阴阳失调、积湿积热、中焦气机不利所致腹胀、腹痛、阳痿、早泄等病症,皆可以此方加减。
Chronic diarrhea refers to diarrhea symptoms that persist or recur for more than 4 weeks.Western medicine treatment usually adopts standardized treatment plans,lacking personalized considerations.Factors such as the physical condition and severity of the condition of different patients can affect the treatment effect,and standardized treatment plans may not meet the needs of all patients.Traditional Chinese medicine has the advantages of personalized treatment,comprehensive regulation,minimal side effects,emphasis on prevention,and long-lasting therapeutic effects in the treatment of chronic diarrhea.This article focuses on the common dialectical classification of chronic diarrhea and explores the diagnosis and treatment of chronic diarrhea,as well as the sources and modified applications of Shanqian Tang.Shanqian Tang is a representative empirical formula of the eight clinical syndromes created by Professor ZHANG Lei,a master of traditional Chinese medicine.It is composed of raw hawthorn,fried hawthorn,raw Plantago seed,and fried Plantago seed,and is specially designed for Yin Yang imbalance,dampness and heat accumulation diseases.The Shanqian Tang formula is a combination of raw and cooked drugs,which has the function of regulating Yin and Yang,eliminating accumulation and stopping diarrhea.In clinical practice,it is effective as a drum for chronic diarrhea with Yin Yang imbalance,dampness and heat accumulation.However,Shanqian Tang has the characteristics of being as effective as a drum.When using Tang clinically,it is not necessary to limit it to the treatment of diarrhea.Any symptoms such as abdominal distension,abdominal pain,impotence,and premature ejaculation caused by imbalance of Yin and Yang,accumulation of dampness and heat,and unfavorable middle Jiao Qi mechanism can be modified with this formula.
