摘要
目的:通过收集整理谢沛霖教授门诊的慢性胃炎患者医案,基于古今医案云平台V 2.3.5,系统分析、总结谢沛霖教授对于慢性胃炎的辨证论治思路及处方用药经验。方法:纳入对象为2020年1月-2020年12月于武汉市中西医结合医院谢沛霖教授中医门诊就诊的慢性胃炎患者,从门诊系统中摘录符合纳入标准的医案信息,基于古今医案云平台对其论治慢性胃炎进行统计分析。结果:对符合纳入标准的143条数据进行分析,得出常见证型为脾虚肝旺、脾胃气虚以及肝胃不和;常用药物为黄连、茯苓、白术、半夏、旋覆花;核心处方为黄连、吴茱萸、柴胡、枳实、赤芍、茯苓、白术。结论:谢沛霖教授认为慢性胃炎脾胃虚弱是基础,气机升降失调为主机,论治时常以疏肝健脾、调畅气机为主。
Objective:By collecting and sorting out the medical records of patients with chronic gastritis in Professor Xie Peilin’s outpatient clinic,based on the ancient and modern medical record cloud platform V2.3.5,systematically analyze and summarize Professor Xie Peilin’s thinking on syndrome differentiation and treatment of chronic gastritis and prescription medication experience.Methods:The subjects were chronic gastritis patients who were treated in the outpatient clinic of Professor Xie Peilin of Wuhan Hospital of Integrated traditional Chinese and Western Medicine from January 2020 to December 2020.The medical record information in accordance with the inclusion criteria was extracted from the outpatient system.Based on the ancient and modern medical records cloud platform,the diagnosis and treatment of chronic gastritis were statistically analyzed.Results:143 pieces of data in accordance with the inclusion criteria were analyzed,and it was concluded that the common syndromes were spleen deficiency and exuberant liver,spleen and stomach qi deficiency and disharmony between liver and stomach,and the common drugs were Coptis chinensis,Poria cocos,Atractylodes macrocephala,Pinellia ternata and Xuanfuhua.Conclusion:Professor Xie Peilin believes that the TCM pathogenesis of chronic gastritis is the weakness of spleen and stomach and the imbalance of qi rise and fall,and the treatment is often based on soothing the liver and invigorating the spleen and regulating qi.
