拔除上颌第一前磨牙正畸治疗后上颌骨开窗和骨开裂的CBCT评价

CBCT evaluation of maxillary fenestration and bone dehiscence after orthodontic treatment of maxillary first premolar extraction

摘要 目的应用锥形束CT(CBCT)评价拔除上颌第一前磨牙正畸治疗后上颌骨开窗和骨开裂状况。方法回顾性抽取2020年1月至2023年1月于平煤神马医疗集团总医院行拔除上颌第一前磨牙正畸治疗的患者80例(480颗前牙), 均行最大支抗内收前牙治疗, 待治疗完成后行CBCT检查, 统计正畸治疗后唇侧和舌侧上颌前牙骨开窗发生率、骨开裂发生率及二者总发生率。结果唇侧右上中切牙骨开裂发生率低于舌侧, 骨开窗发生率高于舌侧(P<0.05)。唇侧左上侧切牙骨开裂发生率低于舌侧(P<0.05)。唇侧右上侧切牙骨开裂发生率、总发生率低于舌侧(P<0.05)。唇侧左上中切牙骨开裂发生率、总发生率低于舌侧(P<0.05)。结论拔除上颌第一前磨牙正畸治疗后, CBCT可见上颌舌侧切牙骨开裂发生率较高, 故治疗中应注意控制内收限度以规避骨开裂。 Objective To evaluate the maxillary fenestration and bone dehiscence after orthodontic treatment of maxillary first premolar extraction by cone beam computed tomography(CBCT).Methods A total of 80 patients(480 anterior teeth)who underwent maxillary first premolar extraction treatment in Pingmei Shenma Medical Group General Hospital from January 2020 to January 2023 were retrospectively selected.All patients received maximum anchorage adduction anterior teeth treatment,and CBCT was performed after the treatment.The incidence of bone fenestration,bone dehiscence and the total incidence of maxillary anterior teeth on the labial and lingual sides after orthodontic treatment were recorded.Results The incidence of bone dehiscence in the right upper central incisor on the labial was lower than that on the lingual,and the incidence of bone fenestration in the right upper central incisor on the labial was higher than that on the lingual(P<0.05).The incidence of bone dehiscence in the left upper incisor on the labial was lower than that on the lingual(P<0.05).The incidence and total incidence of bone dehiscence of the right upper incisor on the labial were lower than those on the lingual(P<0.05).The incidence and total incidence of bone dehiscence in the left upper central incisor on the labial were lower than those on the lingual(P<0.05).Conclusions CBCT showed that the incidence of bone dehiscence of maxillary lingual incisors after orthodontic treatment of maxillary first premolar extraction is high,so attention should be paid to control the adduction limit to avoid bone dehiscence.
