磁共振多参数及3D-ASL序列观察缺血性脑卒中急性期与恢复期的影像学特征变化及其临床意义

Observation of changes in imaging characteristics and clinical significance of ischemic stroke during acute and convalescent stages using multi-parameter magnetic resonance imaging and 3D-ASL seguence

摘要 目的探究磁共振多参数及3D-ASL序列观察缺血性脑卒中急性期与恢复期的影像学特征变化及其临床意义。方法回顾性分析我院2022年2月~2023年1月期间收治的缺血性脑卒中患者60例,其中急性期30例,恢复期30例,急性期作为观察组,恢复期作为对照组;另选择同期前来医院接受体检的健康人群60名,健康人群作为参照组,均给予磁共振多参数(T1WI、T2WI、DWI、SWI)及3D-ASL序列观察,了解健康群体与缺血性脑卒中急性期、恢复期的影像学特征变化,将研究涉及数据纳入统计学表格进行对比分析。结果(1)缺血性脑卒中急性期病灶平均体积与恢复期相比明显要大,对比有统计学意义(P<0.05)。(2)缺血性脑卒中急性期平均血清TNC指标明显高于恢复期(P<0.05),缺血性脑卒中急性期、恢复期平均血清TNC指标均高于参照组(P<0.05)。(3)影像学检测结果显示,60例缺血性脑卒中患者病灶平均体积可根据病情严重程度逐步增大,经有效治疗后处于恢复期阶段病灶体积可不断缩小;3D-ASL序列观察结果看,在针对60例缺血性脑卒中患者缺血半暗带测量脑血流量值可明显看出急性期平均脑血流量平均值明显低于恢复期脑血流量平均值(30<40),患者经治疗后在恢复期脑血流量平均值出现上升表现;在SWI序列上,60例缺血性脑卒中患者有部分恢复期患者病灶内代偿血管与急性期相比有不同程度减少表现。结论磁共振多参数及3D-ASL序列观察缺血性脑卒中急性期与恢复期中具有显著效果,能够协助临床对患者脑血流及病灶情况进行观察,为后续治疗提供充足信息支撑。 Objective To explore the changes in imaging characteristics and clinical significance of multi-parameter magnetic resonance imaging(MRI)and 3D-ASL sequence in the acute and convalescent stages of ischemic stroke.Methods A retrospective analysis was conducted on 60 patients with ischemic stroke admitted to our hospital from February 2022 to January 2023,including 30 in the acute phase and 30 in the recovery phase.The acute phase was used as the observation group and the recovery phase as the control group;Iln addition,60 healthy people who came to the hospital for physical examination during the same period were selected as the reference group,and all of them were subjected to multi-parameter magnetic resonance imaging(T1WI,T2WI,DWI,SWI)and 3D-ASL sequence observation to understand the changes in imaging characteristics between the healthy people and the acute and convalescent stages of ischemic stroke.The data involved in the study were included in statistical tables for comparative analysis.Results(1)The average lesion volume in the acute phase of ischemic stroke was significantly smaller than that in the recovery phase,with a statistically significant difference(P<0.05).(2)The average serum TNC index in the acute stage of ischemic stroke was significantly higher than that in the convalescent stage(P<0.05).(3)The imaging examination results showed that the average volume of the lesions in 60 patients with ischemic stroke could gradually increase according to the severity of the condition,and after effective treatment,the volume of the lesions in the recovery stage could continue to decrease;According to the observation results of 3DASL sequence,the average cerebral blood flow in the ischemic penumbra of 60 patients with ischemic stroke during the acute phase was significantly lower than the average cerebral blood flow in the recovery phase(30<40),and the average cerebral blood flow in the recovery phase increased after treatment;On the SWI sequence,in 60 patients with ischemic stroke,there was a decrease in the
