摘要
目的观察化痰通络开窍方联合针刺治疗缺血性脑卒中后认知障碍的临床疗效.方法选取我院2021年1月—2022年12月收治的60例缺血性脑卒中后认知障碍患者为研究对象.根据就诊顺序进行分组,序号为奇数的30例为观察组,序号为偶数的30例为对照组.对照组在规范化基础治疗的基础上实施针刺治疗,观察组在对照组治疗的基础上给予化痰通络开窍方.对比两组临床疗效、认知功能、日常生活能力、神经元特异性烯醇化酶(NSE)水平及不良反应发生率.结果观察组治疗总有效率为93.33%,明显高于对照组的70.00%,差异有统计学意义(P<0.05).治疗后,观察组简易智力状态检查量表、改良Barthel指数评分高于对照组,NSE水平低于对照组,组间差异有统计学意义(P<0.05).观察组不良反应发生率为6.67%,与对照组的3.33%比较,差异无统计学意义(P>0.05).结论化痰通络开窍方联合针刺可有效提高缺血性脑卒中认知障碍患者的临床效果,并改善认知障碍,提升生活能力,降低NSE水平,且无明显不良反应.
Objective To observe the clinical effect of phlegm and Tongluo enlightenment prescription combined with acupuncture in the treatment of ischemic stroke cognitive impairment.Methods 60 patients with ischemic stroke cognitive impairment admitted from January 2021 to December 2022 were selected as the research subjects.Group the study subjects according to the order of visits,with 30 cases with odd numbers as the observation group and 30 cases with even numbers as the control group.The control group carried out acupuncture treatment on the basis of standardized basic treatment,and the observation group gave a prescription to dissolve phlegm and open up the collaterals on the basis of the treatment of the control group.Compare the clinical efficacy,cognitive function,daily life ability,neuron-specific enolase(NSE)level and the incidence of adverse reactions of the two groups.Results The total treatment efficiency of the observation group was 93.33%,which was significantly higher than 70.00%of the control group,with a statistically significant difference(P<0.05).After treatment,the observation group had higher scores on the Simplified Intelligence State Examination Scale and the Modified Barthel Index,while the NSE level was lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 6.67%,which was not significantly different from 3.33%in the control group(P>0.05).Conclusion The combination of phlegm and retic prescription combined with acupuncture can effectively improve the clinical effect of patients with ischemic stroke cognitiveimpairment,improve cognitive impairment,improve life ability,reduce NSE level,and have no obvious adverse reactions.
