摘要
目的探讨儿童肺炎支原体(MPP)肺泡灌洗液中细胞间黏附分子-1(ICAM-1)表达量与肺炎黏液栓形成的关系。方法本研究选取2022年8月至2023年11月期间在本院住院治疗的80例小儿MPP患儿作为研究对象。根据气管镜下黏液分泌的特点将此80例患儿分为研究组(即黏液栓组)55例,对照组(即非黏液栓组)25例,分析ICAM-1表达量与肺炎黏液栓形成的关系。结果研究组白细胞计数(WBC)、中粒粒细胞比率(N)、C反应蛋白(CRP)、乳酸脱氢酶(LDH)、ICAM-1水平均高于对照组(P<0.05);多因素logistic回归分析显示,WBC、N、CRP、LDH、ICAM-1为影响肺炎黏液栓形成的独立影响因素(P<0.05);研究组肺泡灌洗液中ICAM-1水平与WBC、N、CRP、LDH均呈正相关(P<0.05)。结论MPP患儿疾病越重,患儿体内的炎性反应也越强烈,ICAM-1的表达水平对MPP儿童炎性反应进程的影响是显著的。当其浓度升高时,可能导致肺炎黏液栓的形成。
Objective To investigate the relationship between the expression of intercellular adhesion molecule-1(ICAM-1)in alveolar lavage fluid of mycoplasma pneumoniae and the formation of pneumonia mucus plugs in children.Methods Eighty children with pediatric mycoplasma pneumoniae(MPP)hospitalized in our hospital between August 2022 and November 2023 were selected for this study.These 80 children were divided into the study group(the mucus plug group,55 cases)and the control group(the non-mucus plug group,25 cases)according to the characteristics of mucus secretion under tracheoscopy,and the relationship between ICAM-1 expression and the formation of mucus plugs in pneumonia was analyzed.Results The levels of WBC,N,CRP,LDH,and ICAM-1 in the study group were higher than those in the control group(P<0.05);Multifactorial logistic regression analysis showed that WBC,N,CRP,LDH,and ICAM-1 were independent influences affecting the formation of mucus plugs in pneumonia(P<0.05).The levels of ICAM-1 in the alveolar lavage fluid of the study group were related to WBC,N,CRP,and LDH were positively correlated(P<0.05).Conclusion The more severe the disease in children with MPP,the stronger the inflammatory response in the children′s bodies,and the effect of the expression level of ICAM-1 on the process of inflammatory response in children with MPP is significant.When its concentration is elevated,it may lead to the formation of pneumonia mucus plugs.
