YBX1在肝细胞肝癌中的表达及临床意义

摘要 目的检测Y-盒结合蛋白-1(YBX1)基因(YBX1)在肝细胞肝癌(HCC)和非肿瘤组织中的表达,探讨YBX1在HCC诊断和预后判断中的价值。方法通过癌症基因组图谱(TCGA)数据集进行YBX1表达与临床病理特征的关联,使用KM曲线、Cox回归分析评估YBX1表达在HCC患者中预后,通过基因集富集分析(GSEA)评估YBX1参与HCC发病机制所涉及的生物途径,使用ssGSEA对YBX1基因表达和免疫浸润进行关联分析,进而分析YBX1与免疫浸润的联系。结果共纳入TCGA数据库中424例样本HCC数据,其中癌旁组织样本50例,肿瘤样本374例。HCC中YBX1表达明显高于正常组织(P<0.001)。HCC中YBX1表达的升高与临床分期(P=0.029)、病理等级(P=0.014)显著相关,而在N分期(P=0.089)中则略微显著。根据YBX1表达的中值(6.867),将所有患者分为高表达组和低表达组。与低表达组HCC患者相比,高表达组HCC患者与更差的总生存期(OS)、更差的无进展生存期和更差的疾病特异性生存期相关。单因素及多变量Cox回归进一步分析显示,YBX1表达(HR=2.369,P<0.001)是HCC OS的独立预后因素。此外,YBX1高表达显著富集到G蛋白偶联受体(GPCR)配体结合通路和白细胞介素的反应蛋白信号通路,而且YBX1高表达与Th2细胞浸润水平(R=0.36,P<0.001)、Th7细胞浸润水平(R=-0.29,P<0.001)显著相关。结论YBX1可能是存活率低的HCC的潜在预后指标。GPCR配体结合通路和白细胞介素的反应蛋白信号通路可能是YBX1调节的关键通路。此外,YBX1在HCC中的表达与免疫浸润水平有关。 Objective To detect the expression of Y-box binding protein-1(YBX1)gene(YBX1)in hepatocellular carcinoma(HCC)and non-tumor tissues,and explore the value of YBX1 in diagnosis and prognosis of HCC.Methods The correlation between YBX1 expression and clinicopathological features was carried out by The Cancer Genome Atlas(TCGA)data set.The prognosis of YBX1 expression in HCC patients was evaluated by KM curve and Cox regression analysis.The biological pathways involved in the pathogenesis of HCC were evaluated by gene set enrichment analysis(GSEA).The correlation between YBX1 expression and immune infiltration was analyzed by ssGSEA.Results A total of 424 samples of HCC data in the TCGA database were selected,which included 50 cases of paracancerous tissue samples and 374 of tumor samples.The expression of YBX1 in HCC was significantly higher than that in normal tissues(P<0.001).The increased expression of YBX1 in HCC was significantly correlated with clinical stage(P=0.029)and pathological grade(P=0.014),while it was slightly significant in N stage(P=0.089).According to median value of YBX1 expression(6.867),all of HCC patients were divided into high expression group and low expression group.Compared with low expression group,high expression group was correlated with worse overall survival(OS),progression-free survival and disease-specific survival.The univariate and multivariate Cox regression analysis showed that YBX1 expression(HR=2.369,P<0.001)was an independent prognostic factor for HCC OS.In addition,high expression of YBX1 was significantly enriched in G-protein coupled receptor(GPCR)ligand binding pathway and interleukin-responsive protein signaling pathway,and high expression of YBX1 was significantly correlated with Th2 cell infiltration level(R=0.36,P<0.001)and Th7 cell infiltration level(R=-0.29,P<0.001).Conclusion It is demonstrated that YBX1 may be the potential prognostic indicator for HCC with low survival rate.The GPCR ligand binding pathway and interleukin response protein signaling pathway may be t
