摘要
炎症反应和氧化应激是脑梗死发生的关键病理过程,髓过氧化物酶(myeloperoxidase,MPO)连接这两个过程并参与脑梗死的发生,抑制MPO表达是治疗脑梗死的新靶点。益气、活血、清热解毒类中药含有多种抑制MPO活性的生物成分,具有高效低毒、多通路及多靶点协同作用等优势,可通过抗炎、抗氧化、抗凋亡、抗血栓、抗自噬、抗兴奋性毒性、改善脑循环及促进神经修复再生等途径发挥神经保护作用,减轻缺血脑组织的神经损伤。目前,关于MPO与心血管系统疾病的研究较多,但神经病学领域鲜有基于MPO抗炎、抗氧化作用的临床观察研究,今后可将网络药理学与体内外实验联合验证中药有效成分的作用机制和疗效,同时加强大样本的随机对照临床研究,为神经保护剂的开发提供新思路,以期提高脑梗死的临床诊疗水平。
Inflammatory response and oxidative stress are key pathological processes in the occurrence of cerebral infarction.Myeloperoxidase(MPO)connects these two processes and participates in the occurrence of cerebral infarction.Inhibiting MPO expression is a new target for treating cerebral infarction.Chinese herbal medicine that benefiting Qi,activating blood,clearing Heat and resolving toxin contain various biological components that inhibit MPO activity.They have advantages such as high efficiency,low toxicity,multi pathway and multi target synergistic effects.They can exert neuroprotective effects and reduce nerve damage in ischemic brain tissue through anti-inflammatory,anti-oxidation,anti-apoptosis,anti-thrombosis,anti-autophagy,anti-excitotoxicity,improving cerebral circulation as well as promoting nerve repair and regeneration.At present,many studies are focused on the relationship between MPO and cardiovascular diseases,while there are few clinical observation studies based on the anti-inflammatory and antioxidant effects of MPO in the field of neurology.In the future,network pharmacology can be combined with in vitro or in vivo experiments to verify the mechanism and efficacy of effective ingredients in traditional Chinese medicine.At the same time,large-scale randomized controlled clinical studies can be strengthened to provide new ideas for the development of neuroprotective agents,to improve the clinical diagnosis and treatment of cerebral infarction.
