富血小板血浆治疗烧伤创面有效性与安全性的Meta分析

摘要 目的通过Meta分析系统评价富血小板血浆促进烧伤创面愈合的作用。方法使用计算机对万方、中国知网、维普、PubMed、Embase、Cochrane Library、Web of Science等中外数据库进行检索,中文检索词包含“富血小板血浆”“烧伤”“烧伤创面”等,英文检索词包括“platelet rich plasma”“burn”“burn wound”等,检索建库至2024年1月31日收录的临床中关于使用富血小板血浆治疗烧伤创面的试验文章,依据纳入与排除标准,对纳入文献进行查重、筛选、质量评价、数据提取后,使用Review Manager 5.4.1软件进行Meta分析。结果最终纳入28篇文献,2279例患者,对结局指标进行Meta分析后显示:试验组的创面愈合时间[MD=-4.83,95%CI(-5.65,-4.02),P<0.00001]、创面细菌培养阳性率[OR=0.42,95%CI(0.42,0.75),P=0.003]、渗湿纱布层数[MD=-5.09,95%CI(-6.42,-3.77),P<0.00001]和1年后VSS评分[MD=-0.77,95%CI(-1.00,-0.55),P<0.00001]均低于对照组;试验组的治疗有效率[OR=6.03,95%CI(3.33,10.92),P<0.00001]和第1周[MD=8.43,95%CI(7.20,9.66),P<0.00001]、第2周[MD=13.64,95%CI(8.73,18.56),P<0.00001]、第3周[MD=15.84,95%CI(15.05,16.62),P<0.00001]的创面愈合率均高于对照组;二者在移植物黏附率[MD=24.37,95%CI(-19.10,67.84),P=0.27]和过敏反应发生率[OR=0.54,95%CI(-0.18,1.65),P=0.28]方面无显著差异。结论富血小板血浆在烧伤创面治疗过程中促进创面愈合,提升治疗有效率和创面愈合率,减少渗出和创面细菌培养阳性率,远期VSS评分更好;对移植物黏附率和过敏反应发生率无明显影响。 Objective To systematically evaluate the effect of platelet-rich plasma on burn wound healing through meta-analysis.Methods The Chinese and foreign databases such as Wanfang,CNKI,VIP,PubMed,Embase,Cochrane Library,Web of Science,etc.were searched by computer,and the search terms included"platelet-rich plasma","burn","burn wound",etc.in Chinese,and"platelet rich plasma","bum","burn wound",etc.in English.The clinical trial articles on the use of platelet-rich plasma in the treatment of burn wounds were searched from the establishment of the database to January 31,2024,and the included studies were checked for duplication,screened,quality evaluation and data extracted according to the inclusion and exclusion criteria,and meta-analysis was performed using Review Manager 5.4.1 software.Results A total of 28 articles and 2279 patients were included,and the meta-analysis of the outcome measures showed that the wound healing time[MD=_4.83,95%CI(-5.65,-4.02),P<0.00001],positive rate of wound bacterial culture[OR=0.42,95%CI(0.42,0.75),P=0.003],the number of wet gauze layers[MD=-5.09,95%CI(-6.42,-3.77),P<0.00001]and the VSS score after 1 year[MD=-0.77,95%CI(-1.00,-0.55),P<0.00001]in the experimental group were lower than those in the control group.The effective rate of treatment[OR=6.03,95%CI(3.33,10.92),P<0.00001]and the wound healing rate of the experimental group at the 1st[MD=8.43,95%CI(7.20,9.66),P<0.00001],2nd[MD=13.64,95%CI(8.73,18.56),P<0.00001]and 3rd[MD=15.84,95%CI(15.05,16.62),P<0.00001]weeks were higher than those in the control group.There was no significant difference in graft take rate[MD=24.37,95%CI(-19.10,67.84),P=0.27]or incidence of allergic reactions[OR=0.54,95%CI(~0.18,1.65),P=0.28].Conclusion Platelet-rich plasma can promote wound healing,improve the effective rate and wound healing rate,reduce the exudation and positive rate of wound bacterial culture,and have a better long-term VSS score in the treatment of burn wounds.There was no significant effect on graft adhesion rate and incidence of allergic r
