摘要
目的探讨超声心动图评估卵圆孔未闭患者手术前后的心功能变化研究。方法选择2021年2月—2022年3月本院收治的98例卵圆孔未闭患者为研究对象,所有患者均进行超声心电图检查,观察常规二维超声图像与三维超声心动图检出卵圆孔未闭情况,常规二维超声图像与三维超声心电图测量卵圆孔未闭卵圆孔宽度及宽度,分析卵圆孔介入手术成功率及术后情况,统计术后卵圆孔未闭患者心功能变化情况比较。结果以临床综合诊断结果为金标准,三维超声心动图诊断卵圆孔未闭的准确率为89.71%、灵敏度为93.44%、特异度为100%。采用Kappa一致性检验,Kappa值为0.97,一致性较佳。临床综合诊断与三维超声心电图测量的卵圆孔宽度、长度比较,差异均无统计学意义(P>0.05)。68例卵圆孔未闭患者均成功完成卵圆孔介入手术,成功率达100%,封堵器部位固定,形态良好,房间隔无分流,同时观察患者无并发症。术前及术后,68例卵圆孔未闭患者心功能并未发生明显变化,差异不存在统计学意义(P>0.05)。结论超声心动图评估卵圆孔未闭患者手术前后的心功能变化情况,超声心动图可应用于卵圆孔未闭患者诊断及预后评估,可在临床推广应用。
Objective To investigate the variation of cardiac function before and after surgery in patients with patent foramen ovale assessed by echocardiography.Methods 98 patients suffering from patent foramen ovale who were admitted to our hospital from February 2021 to March 2022 were enrolled as study subjects.All patients underwent echocardiographic examination,and observed the detection of patent foramen ovale by conventional two-dimensional ultrasonic image and three-dimensional echocardiography,and the width and width of patent foramen ovale were measured by conventional two-dimensional ultrasound image and three-dimensional echocardiography.To analyze the success rate of interventional foramen ovale operation and the postoperative situation,and compare the changes of cardiac function in patients with patent foramen ovale.Results Clinical comprehensive diagnosis results were taken as the gold standard,the accuracy of three-dimensional echocardiography in diagnosing patent foramina ovale was 89.71%,the sensitivity was 93.44%,and the specificity was 100%.Kappa consistency test was adopted,and the Kappa value was 0.97,indicating good consistency.The width and length of foramen ovale measured by clinical comprehensive diagnosis and three-dimensional echocardiography were compared,and the difference as statistically significant(P>0.05).All 68 patients with patent foramen ovale successfully completed the intervention surgery for foramen ovale,the success rate was 100%,the occluders were fixed,the shape was good,the atrial septum was not shunt,and no complications were observed.Before and after surgery,there was no significant change in cardiac function in 68 patients with patent foramen ovale(P>0.05).Conclusions Echocardiography can be used to evaluate the changes of cardiac function in patients with patent foramen ovale before and after surgery,and could be popularized and applied in clinic.
