超声心动图用于肺心病患者诊断的临床价值分析

摘要 目的探讨肺心病患者应用超声心动图诊断的临床价值。方法选取2019年9月至2021年6月于阳谷县人民医院经X线透视下心导管检查确诊的86例肺心病患者作为观察组,选取同期收治的64例疑似肺心病但经X线透视下心导管检查阴性者作为对照组。所有患者均接受超声心动图检查、心电图检查和心导管检查。比较两组超声心动图指标、右心功能指标,并分析超声心动图与心电图的诊断价值。结果观察组右心室前壁厚度(RVAW)、右心室游离壁舒张末期厚度(RVDT)均厚于对照组,心室前后径(RV)、右心房横径(RA)、右心室舒张末期横径(RVDD)均长于对照组,射血时间(ET)短于对照组,Tei指数高于对照组,差异有统计学意义(P<0.05)。超声心动图诊断的灵敏度(81.40%)、特异度(87.50%)、准确度(84.00%)均高于对照组的65.12%、71.88%、68.00%,差异有统计学意义(P<0.05),且AUC更高,为0.764,95%CI为0.648~0.861。结论超声心动图能较好地显示肺心病患者右心结构和功能的变化,灵敏度、特异度和准确度更高,临床诊断价值更高,值得临床推广应用。 Objective To explore the clinical value of echocardiography in the diagnosis of patients with pulmonary heart disease.Methods A total of 86 patients were diagnosed with pulmonary heart disease by X-ray fluoroscopic cardiac catheterization from September 2019 to June 2021 in Yanggu County People's Hospital were selected as the observation group,and 64 patients suspected with pulmonary heart disease but negative by X-ray fluoroscopic cardiac catheterization in our hospital during the same period were selected as the control group.All the patients received echocardiography,electro-cardiogram(ECG)and cardiac catheterization.The echocardiography indicators and right heart function indicators were compared between the two groups,and the diagnostic value of echocardiography and electrocardiogram was analyzed.Results The right ventricular anterior wall thickness(RVAW)and right ventricular free wall end-diastolic thickness(RVDT)in the observation group were thicker than those in the control group,anterior and posterior ventricular diameter(RV),right atrial transverse diameter(RA)and right ventricular end-diastolic transverse diameter(RVDD)were longer than those in the control group,ejection time(ET)was shorter than that in the control group,and Tei index was higher than that in the control group,the differences were statistically significant(P<0.05).The sensitivity(81.40%),specificity(87.50%),and accuracy(84.00%)of echocardiography in the diagnosis were higher than 65.12%,71.88%,and 68.00%in the control group,the differences were statistically significant(P<0.05).Moreover,AUC(0.764)of echocardiography was higher,and the 95%CI was 0.648-0.861.Conclusion Echocardiography can better show the changes of right heart structure and function in patients with pulmonale,it has higher sensitivity,specificity and accuracy,higher clinical diagnostic value,which is worthy of clinical promotion and application.
