摘要
目的:从宏代谢层面阐释大肠癌及其常见中医证型的生物学特征。方法:收集107例大肠癌湿热证、肝肾阴虚证、脾肾阳虚证患者和35名健康人晨便,通过液相色谱质谱联用技术及多元统计方法分析粪便细菌代谢物。结果:肠癌组粪便中17种代谢小分子[氨基酸、有机酸、(短链)脂肪酸、肉碱类代谢物]异于健康组;湿热证粪便胆汁酸明显异于健康及其他证候人群,其中次级胆汁酸显著降低,初级胆汁酸显著增加;肝肾阴虚证和脾肾阳虚证两组粪便长链不饱和脂肪酸水平升高而有机酸水平降低。结论:大肠癌患者粪便宏代谢表型明显异于健康人,大肠癌常见证候的粪便宏代谢特征也不尽相同,其中湿热证以胆汁酸代谢异常为特征,肝肾阴虚证的不饱和脂肪酸异常代谢尤为明显,而脾肾阳虚证的食物发酵、营养供应不足。
Objective:To characterize biological features of colorectal cancer(CRC)and its traditional Chinese medicine(TCM)pattern from the meta-metabolomic perspective.Methods:Morning stool samples from 107 CRC patients with dampness-heat(DH),liver kidney yin deficiency(LKYD),and spleen kidney yang deficiency(SKYD)syndromes and 35 healthy controls(HC)were collected to profile fecal meta-metabolome based on UPLC-MS/MS and multivariate statistical methods.Results:A total of 17 metabolites categorized as amino acids,organic acids,(short-chain)fatty acids were identified,and carnitines significantly altered in CRC patients relative to healthy controls.Compared with either controls or other TCM pattern groups,the DH pattern was featured with changed bile acids,of which primary bile salts were raised while secondary bile acids were reduced.Moreover,higher levels of unsaturated long chain fatty acids were specifically shown in the LKYD group,and organic acids were insufficient in the SKYD group.Conclusion:Fecal metametabolome of CRC differes from that of HC.The meta-metabolome characteristics of feces in common syndromes of CRC are also different,among which DH syndrome is featured by bile acid dysmetabolism,LKYD syndrome has abnormal unsaturated fatty acid metabolism,while SKYD syndrome is insufficient in intraluminal food fermentation and nutrient supply.
