摘要
目的提高肺结核发现报告质量,促进肺结核患者及早接受规范化诊疗管理。方法基于医院信息系统(HIS)开发“公共卫生预警监测管理系统”,建立传染病发现及报告组织管理体系,完善肺结核等传染病报告管理制度,分析2017—2019年监测数据以评价预警监测管理系统对肺结核发现报告质量的影响。结果实现了预警监测系统建设。与系统使用第一年的2017年相比,2018年和2019年临床医师在缺乏痰检阳性及影像学阳性等证据下通过诊断流程诊断肺结核后发出预警的比例明显升高(χ^(2)=78.989,P<0.001),且临床医师主动响应上报比例明显提高(χ^(2)=211.136,P<0.001)。结论充分利用信息系统的预警监测功能可以提高临床医师结核病防治意识、临床诊断能力、病例报告的及时性和报告质量,从而促进肺结核诊断及规范管理。
Objective To improve the quality of tuberculosis detection report and to promote the standardize diagnosis and treatment management early.Methods The Public Health Early Warning,Monitoring and Management System was developed based on the hospital HIS system,the infectious disease detection and reporting organization and management system was established,the reporting and management system of tuberculosis and other infectious diseases was improved,the monitoring data was analyze from 2017 to 2019 to evaluate the impact of early warning and monitoring management systems on the quality of tuberculosis detection and reporting.Results The construction of an early warning and monitoring system was implemented.Compared with the first year of system use in 2017,the proportion of clinical physicians issuing warnings after diagnosing pulmonary tuberculosis through diagnostic procedures in 2018 and 2019 was significantly higher(χ^(2)=78.989,P<0.001),and the proportion of clinical physicians actively responding to reports was significantly higher(χ^(2)=211.136,P<0.001).Conclusions Sufficient use of the early warning and monitoring function of the information system could greatly improve the quality of tuberculosis detection and reporting,promote the clinical diagnostic ability,timeliness and quality of case reports,so as to promote the diagnosis and standardized management of pulmonary tuberculosis.
