摘要
目的研究腹腔镜精索静脉结扎联合中药治疗精索静脉曲张伴少、弱精子症的临床疗效。方法收集本院就诊并手术治疗的双侧精索静脉曲张伴少、弱精子症的患者72例,随机分为观察组(36例)和对照组(36例)。两组均行腹腔镜精索静脉高位结扎手术,观察组术后口服中药。两组术后8周、12周、24周复查精液常规检查。结果两组患者术后无复发。观察组与对照组术后第8周、第12周、第24周的精子浓度、向前运动精子率(PR)、非向前运动精子率(NR)+PR较术前均有所改善(P<0.05)。在精子浓度及PR方面,观察组术后第8周、第12周、第24周均优于同时期对照组,差异有统计学意义(P<0.05);在精子总活力(PR+NR)方面,两组在第8周时差异无统计学意义(P>0.05),在第12周、第24周时,观察组优于对照组,差异有统计学意义(P<0.05)。结论腹腔镜精索静脉高位结扎术联合中药治疗精索静脉曲张伴少、弱精子症的临床疗效显著,优于单纯手术治疗,值得临床推广。
Objective To study the clinical effect of laparoscopic spermatic vein ligation combined with traditional Chinese medicine on varicocele with oligospermia and asthenospermia.Methods 72 patients with bilateral varicocele accompanied by oligospermia and asthenospermia who received surgical treatment in a hospital were collected and randomly divided into the experimental group(36 cases)and the control group(36 cases).Both groups underwent laparoscopic high ligation of the spermatic vein,and the experimental group took Chinese medicine orally after the operation.The two groups were reexamined for semen routine examination at 8,12,and 24 weeks after surgery.Results There was no recurrence in the two groups.The sperm concentration,Progressive Motility Rate(PR),and Non-Progressive Motility Rate(NR)+PR of the experimental and control groups were improved at 8,12,and 24 weeks after surgery(P<0.05).In terms of sperm concentration and PR,the experimental group was better than the control group at 8,12,and 24 weeks after surgery,the difference was statistically significant(P<0.05).In terms of total sperm motility[PR+NR],there was no statistical difference between the two groups in the 8th week,but in the 12th and 24th week,the experimental group was better than the control group,the difference was statistically significant.Conclusion Laparoscopic high ligation of the spermatic vein combined with traditional Chinese medicine in the treatment of varicocele with oligospermia and asthenospermia has significant clinical efficacy,is superior to surgical treatment alone,and is worthy of clinical promotion.
